滋阴泻火方治疗女童ICPP疗效及对血清25(OH)D、IGF-1的影响

摘　　要：目的:探讨滋阴泻火方治疗女童特发性中枢性性早熟(ICPP)疗效及对血清25-羟基维生素D[25(OH)D]、胰岛素样生长因子(IGF)-1的影响。方法:选取2021年1月~2024年1月期间于本院收治的80例ICPP女童并对其以随机数表法分为对照组(n=40)与观察组(n=40)。对照组常规西药[促性腺激素释放激素类似物(GnRHa)]治疗,观察组在此基础上予以滋阴泻火方治疗,均治疗3个月。对比两组疗效、中医证候积分、盆腔发育情况(卵巢容积、子宫容积、卵泡直径)、性激素[雌二醇(E_(2))、促黄体素(LH)、促卵泡素(FSH)]、血清25(OH)D、IGF-1水平及不良反应。结果:与对照组的80.00%相比,观察组95.00%总有效率更高(P<0.05)。与对照组相比,治疗后观察组中医证候积分、卵巢与子宫容积、卵泡直径均更低(P<0.05)。观察组治疗后血清E_(2)、LH、FSH、IGF-1水平均较对照组更低,血清25(OH)D水平较对照组更高(P<0.05)。两组治疗期间均无明显不良反应发生。结论:滋阴泻火方治疗女童ICPP有利于提高临床疗效,降低性激素水平,改善盆腔发育情况及血清25(OH)D、IGF-1水平,且具安全性。Objective:To explore the curative effect of Ziyin Xiehuo prescription and its influences on serum 25-hydroxyvitamin D[25(OH)D]and insulin-like growth factor(IGF)-1 in girls with idiopathic central precocious puberty(ICPP).Methods:A total of 80 girls with ICPP admitted to the hospital were enrolled between January 2021 and January 2024.According to random number table method,they were divided into control group(n=40)and observation group(n=40).The control group was treated with routine western medicine[gonadotropin-releasing hormone analog(GnRHa)],while observation group was treated with Ziyin Xiehuo prescription on basis of control group for 3 months.The curative effect,scores of TCM syndromes,pelvic development(ovarian volume,uterine volume,follicle diameter),sex hormones[estradiol(E_(2)),luteinizing hormone(LH),follicle-stimulating hormone(FSH)],serum 25(OH)D,IGF-1 and adverse reactions were compared between the two groups.Results:The total effective rate in the observation group was 95.00%,compared with 80.00%in the control group(P<0.05).After treatment,scores of TCM syndromes,ovarian volume,uterine volume and follicle diameter in observation group were lower(P<0.05).Compared with the control group,the levels of serum E_(2),LH,FSH and IGF-1 were lower in the observation group after treatment,and the level of 25(OH)D was higher(P<0.05).There were no significant adverse reactions in either groups during treatment.Conclusion:Ziyin Xiehuo prescription is beneficial to improve clinical curative effect,reduce levels of sex hormones,improve pelvic development,increase levels of serum 25(OH)D and IGF-1 in ICPP girls,which has certain safety.
