除湿胃苓汤结合他克莫司软膏外用对脾胃湿热型脂溢性皮炎患者疗效及皮肤屏障功能的影响

摘　　要：目的:探究除湿胃苓汤结合他克莫司软膏外用对脾胃湿热型脂溢性皮炎患者的作用及影响。方法:选取2022年1月至2023年6月周口市中医院脾胃湿热型脂溢性皮炎患者为对象,共106例。随机抽签法分组,A组(n=53)、B组(n=53)。A组选用他克莫司软膏外用治疗,B组选择他克莫司软膏联合除湿胃苓汤治疗。对不同方法治疗后取得的疗效进行分析。结果:A组总有效率81.13%,B组总有效率96.23%,A组低于B组,差异有统计学意义(P<0.05);A组脱屑和皮肤瘙痒、红斑和皮损面积症状评分高于B组,差异有统计学意义(P<0.05);治疗后,A组尿黄和疲劳、口渴便干和纳差症状积分高于B组,差异有统计学意义(P<0.05);A组IFN-γ、TNF-α水平高于B组,IL-10水平低于B组,差异有统计学意义(P<0.05)。A组pH值和皮肤含水量、皮脂分泌率和经皮表水分流失占比均高于B组,差异有统计学意义(P<0.05)。结论:对脾胃湿热型脂溢性皮炎者行他克莫司软膏联合除湿胃苓汤治疗,效果理想,可以缓解症状,改善皮肤屏障功能,减轻炎性反应。Objective:To explore the effect of deshi Weiling decoction combined with tacrolimus ointment on patients with spleen-stomach damp-heat seborrheic dermatitis.Methods:106 patients with spleen stomach damp heat type seborrheic dermatitis who visited Zhoukou Traditional Chinese Medicine Hospital were selected as the study subjects(study period:January 2022 to June 2023).Randomly draw lots to group A(n value 53)and B(n value 53).Group A was treated with topical application of tacrolimus ointment,while Group B was treated with tacrolimus ointment+dehumidifying stomach ling decoction.Analyze the effects of different treatment methods.Results:The total effective rate of group A was 81.13%,the total effective rate of group B was 96.23%,which was lower in group A than in group B,and the results were statistically significant(P<0.05).The symptom scores of desquamation,pruritus,erythema and skin lesion area in group A were higher than those in group B,and the statistical results were significant(P<0.05).After treatment,the scores of yellow urine,fatigue,thirst and dry stool and poor appetite in group A were higher than those in group B,and the statistical results were significant(P<0.05).the levels of IFN-γand TNF-αin group A were higher than those in group B,and the levels of IL-10 were lower than those in group B,the statistical results were significant(P<0.05).The PH value,skin water content,sebum secretion rate and transcutaneous surface water loss ratio of group A were higher than those of group B,and the statistical results were significant(P<0.05).Conclusion:The effect of Tacrolimus ointment plus Dehumidified-weiling decoction on seborrheic dermatitis of spleen and stomach is ideal,the symptoms are relieved,the skin barrier function is improved and the inflammatory response is alleviated.
