摘　　要：胰腺癌作为消化道肿瘤中恶性程度最高、预后最差的肿瘤之一,因多数患者在初诊时即已处于晚期,丧失根治性手术切除的机会。随着对胰腺癌发病机制及治疗策略研究的深入,治疗模式已从以手术为主逐步转向多学科综合治疗。其中,术前新辅助治疗已成为交界可切除和局部进展期胰腺癌的优选方案。在此背景下,内镜技术在胰腺癌的诊疗中发挥着至关重要的作用。超声内镜(EUS)已成为胰腺癌诊断及治疗的重要手段,尤其EUS引导下穿刺活检及局部治疗可为患者提供精准的病理学诊断与微创干预,而EUS引导的胆道介入治疗则是解决胰腺癌引起的胆道梗阻的高阶治疗方法。本文系统回顾了胰腺癌新辅助治疗背景下EUS诊疗的最新进展,以期为临床提供参考。Pancreatic cancer is one of the most malignant tumors with the worst prognosis in digestive system.Because most patients are already in the advanced stage when they are first diagnosed,they lose the opportunity of radical surgical resection.With the deepening of the research on the pathogenesis and treatment strategy of pancreatic cancer,the treatment mode has gradually shifted from surgery to multidisciplinary comprehensive treatments.Among them,preoperative neoadjuvant therapy has become the preferred protocol for borderline resectable and locally advanced pancreatic cancer.In this context,endoscopy plays a crucial role in the diagnosis and treatment of pancreatic cancer.Therefore,endoscopic ultrasound(EUS)has become an important means for the diagnosis and treatment of pancreatic cancer.In particular,EUS-guided puncture biopsy and local treatment can provide patients with accurate pathological diagnosis and minimally invasive intervention,Moreover,EUS-guided biliary intervention treatment is an advanced treatment method to resolve biliary obstruction caused by pancreatic cancer.This paper systematically reviews the latest progress of EUS diagnosis and treatment based on the neoadjuvant therapy for pancreatic cancer.It is expected to provide the clinical references.
