白药膏外敷联合西药治疗湿热蕴结型急性痛风性关节炎临床研究

摘　　要：目的:观察白药膏外敷联合西药治疗湿热蕴结型急性痛风性关节炎(GA)的临床疗效及安全性。方法:选取64例湿热蕴结型急性GA患者,采用随机数字表法分为治疗组和对照组各32例。2组研究过程中各剔除2例。对照组给予西药(双氯芬酸钠、碳酸氢钠、奥美拉唑)治疗,治疗组在对照组基础上联合白药膏外敷治疗。2组均连续治疗2周。比较2组临床疗效、症状评分及炎症指标[白细胞计数(WBC)、血沉(ESR)、C-反应蛋白(CRP)]水平,评价治疗安全性。结果:治疗2周后,治愈率治疗组50.00%,高于对照组23.33%(P<0.05)。治疗2周后,2组关节肿胀、疼痛、活动受限评分均较治疗前降低,治疗组上述3项症状评分均低于对照组(P<0.05)。治疗2周后,2组WBC、ESR、CRP水平均较治疗前降低(P<0.05),治疗组WBC、ESR水平均低于对照组(P<0.05);2组CRP水平比较,差异无统计学意义(P>0.05)。不良反应发生率治疗组13.33%,对照组16.67%,2组比较,差异无统计学意义(P>0.05)。结论:白药膏外敷联合西药治疗湿热蕴结型急性GA疗效较好,可改善患者关节疼痛、肿胀、活动受限等症状,降低炎症指标水平,且安全性好。Objective:To observe the clinical effect of White Ointment combined with western medicine on acute gouty arthritis(GA)of damp-heat accumulation type and its safety.Methods:A total of 64 cases of patients with acute GA with damp-heat accumulation type were selected and divided into the treatment group and the control group by random number table method,with 32 cases in each group.Two cases were excluded in each group during the study.The control group was treated with western medicine(Diclofenac Sodium,Sodium Bicarbonate,Omeprazole),and the treatment group was combined with White Ointment based on the treatment of the control group.Both groups were treated for two weeks continuously.The clinical effects,symptom scores and inflammatory indexes[white blood cell count(WBC),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)]were compared between the two groups,and the safety of treatment was evaluated.Results:After two-week treatment,the cure rate in the treatment group was 50.00%,higher than that of 23.33%in the control group(P<0.05).After two-week treatment,the scores of joint swelling,pain and limited activity in both groups were decreased when compared with those before treatment(P<0.05),and the above three symptom scores in the treatment group were lower than those in the control group(P<0.05).After two-week treatment,levels of WBC,ESR and CRP in both groups were decreased when compared with those before treatment(P<0.05),and levels of WBC and ESR in the treatment group were lower than those in the control group(P<0.05).There was no significant difference in levels of CRP between the two groups(P>0.05).The incidence of adverse reactions was 13.33%in the treatment group and 16.67%in the control group,there being no significant difference between the two groups(P>0.05).Conclusion:White Ointment combined with western medicine has a great therapeutic effect on acute GA of damp-heat accumulation type,which can relieve the symptoms of joint pain,swelling and limited activity and reduce the levels of inflammati
