基于脑-心-肾理论应用微针刀联合针刺调神治疗女性压力性尿失禁临床研究

摘　　要：目的:观察在脑-心-肾理论指导下微针刀结合针刺调神治疗女性压力性尿失禁(SUI)的临床疗效。方法:选取90例女性SUI患者,按随机数字表法分为微针刀联合针刺组、针刺组、盆底肌康复组各30例。比较3组临床疗效,以及治疗前后1 h漏尿量、72 h漏尿次数、焦虑抑郁压力量表(DASS-21)评分变化。结果:微针刀联合针刺组、针刺组、盆底康复组总有效率分别为96.67%、83.33%、73.33%,经秩和检验,差异有统计学意义(P<0.05)。治疗后,3组1 h漏尿量均较治疗前减少(P<0.05);且微针刀联合针刺组1 h漏尿量均少于针刺组及盆底康复组(P<0.05),而针刺组与盆底康复组比较,差异无统计学意义(P>0.05)。治疗后,3组72 h漏尿次数均较治疗前减少(P<0.05),且微针刀联合针刺组72 h漏尿次数少于针刺组及盆底康复组(P<0.05);而针刺组与盆底康复组比较,差异无统计学意义(P>0.05)。治疗后,3组焦虑、抑郁、压力评分及总分均较治疗前降低(P<0.05),且微针刀联合针刺组、针刺组上述各项评分均低于盆底康复组(P<0.05);而微针刀联合针刺组与针刺组各项评分比较,差异无统计学意义(P>0.05)。结论:微针刀松解结合针刺调神治疗女性SUI疗效更显著,能有效减少漏尿量及漏尿次数,改善患者精神情绪。Objective:To observe the clinical effect of the microneedle knife combined with acupuncture to regulate the mind in the treatment of female stress urinary incontinence(SUI)based on the brain-heart-kidney theory.Methods:A total of 90 cases of female SUI patients were selected and randomly divided into three groups:the microneedle knife and acupuncture group,the acupuncture group,and the pelvic floor muscle rehabilitation group,with 30 cases in each group.Clinical effects were compared among the three groups.The changes in 1-hour urine leakage volume,72-hour urine leakage frequency,and the scores of the Depression Anxiety Stress Scales-21(DASS-21)were compared among the three groups before and after treatment.Results:The total effective rates in the microneedle knife and acupuncture group,the acupuncture group,and the pelvic floor rehabilitation group were 96.67%,83.33%,and 73.33%,respectively,the difference being significant tested by chi-square test(P<0.05).After treatment,the 1-hour urine leakage volume in the three groups was decreased when compared with that before treatment(P<0.05);the 1-hour urine leakage volume in the microneedle knife and acupuncture group was less than that in the acupuncture group and the pelvic floor rehabilitation group(P<0.05),while there was no significant difference between the acupuncture group and the pelvic floor rehabilitation group(P>0.05).After treatment,the 72-hour urine leakage frequency in the three groups was decreased when compared with that before treatment(P<0.05),and the 72-hour urine leakage frequency in the microneedle knife and acupuncture group was less than that in the acupuncture group and the pelvic floor rehabilitation group;there was no significant difference between the acupuncture group and the pelvic floor rehabilitation group(P>0.05).After treatment,the scores of anxiety,depression and pressure,as well as the total scores in the three groups were reduced when compared with those before treatment(P<0.05),and the above scores in the microneedle knife and acu
