摘　　要：中风是老年人常见的疾病,具有不同程度的残疾后遗症。文章提出针刀三级论治:一级辨局部症状,从“以痛为腧”到“以不通为腧”辨治;二级辨应力传导的立体面,从传统辨静息态针刺腧穴到辨运动解剖病理变化的治疗;三级辨神经发病机制治疗,从督脉、膀胱经及华佗夹脊穴附近部位着手,重视整体观,以求本溯源。文章详细探讨了针刀三级论治中风痉挛性偏瘫的机理,为临床针刀治疗中风提供诊疗思路,并实施和推广具有中医特色的针刀技术。Stroke is a common disease among the elderly,with different degrees of disability sequelae.This paper proposes three-level treatment of acupotomy.The first level:acupotomy treatment is to differentiate local syndrome,that is differentiation from‘pain as acupoints’to‘blocked as acupoints’;Second level:to differentiate three-dimensional plane of stress conduction,that is differentiation from the static traditional acupoints to sports anatomy pathology;The third level:to differentiate the neural pathogenesis,take the needling sites near the location of Du meridian,bladder meridian and Huatuo Jiaji points,try to trace the source of pathology by holistic view.Taking as an example,this paper discussed the mechanism of three-level treatment of acupotomy for stroke spastic hemiplegia in detail,to provide diagnosis and treatment ideas for the clinical acupotomy treatment of stroke spastic hemiplegia,and implement and promote the acupotomology with traditional Chinese medicine characteristics.
