摘 要:目的:研究推拿对2型糖尿病(T2DM)大鼠血糖的调控改善胰岛素抵抗(IR)状态以及对骨骼肌超微结构的影响。方法:清洁级Wistar雄性大鼠30只随机分为空白组、模型组、药物组、推拿组,每组6只。空白组和模型组不予干预,药物组盐酸二甲双胍灌胃,推拿组推拿手法干预。干预8周。记录治疗前后空腹血糖(FBG)、空腹胰岛素(FINS)、甘油三脂(TG)、总胆固醇(TC)等指标,计算胰岛素抵抗指数(HOMA-IR)。PAS染色法观察各组大鼠肌糖原水平;HE染色、透射电镜观察各组大鼠腓肠肌的病理变化。Western Blot检测骨骼肌葡萄糖转运体4(GLUT4)蛋白表达。结果:与空白组比较,模型组大鼠FBG、FINS、HOMA-IR、TC、TG水平显著升高(P<0.01),GLUT4蛋白表达显著降低(P<0.01);与模型组比较,推拿组与药物组均能够显著降低大鼠FBG、FINS、HOMA-IR(P<0.01),以及TC、TG水平(P<0.05,P<0.01),大鼠骨骼肌的糖原合成明显增加,GLUT4蛋白表达显著增加(P<0.01),肌肉损伤程度改善。结论:推拿可能通过提高肌肉中GLUT4蛋白表达减轻T2DM大鼠IR进而改善T2DM大鼠的骨骼肌病变程度,促进损伤肌肉恢复。Objective:To explore the effect of massage on blood glucose regulation and insulin resistance in type 2 diabetes mellitus(T2DM)rats and the ultrastructure of skeletal muscle.Methods:Thirty Wistar male rats with clean grade were divided into a blank group,model group,metformin group and massage group,6 in each group.The blank group and model group were not intervened,the drug group was given metformin hydrochloride intragastric administration,and the massage group was treated with massage manipulation,for 8 weeks.Fasting blood glucose(FBG),fasting serum insulin(FINS),triglyceride(TG)and total cholesterol(TC)before and after treatment were recorded,and insulin resistance index(HOMA-IR)was calculated,and the muscle glycogen level of rats in each group was observed by PAS staining.The pathological changes of gastrocnemius were observed by HE staining and transmission electron microscopy.GLUT4 protein expression was detected by Western Blot.Results:Compared with the blank group,the level of FBG,FINS,HOMA-IR,TC and TG in the model group were significantly increased(P<0.01)and GLUT4 protein expression in skeletal muscle was significantly decreased(P<0.01).Compared with the model group,the pathological condition of rats in the massage group and metformin group was improved after treatment,and the FBG,FINS,HOMA-IR(P<0.01).as well as the level of TC,TG(P<0.05,P<0.01),the glycogen synthesis in skeletal muscle of the massage group was significantly increased.GLUT4 protein in skeletal muscle of massage group was significantly increased(P<0.01),the degree of skeletal muscle injury was improved.Conclusion:Massage manipulation may reduce insulin resistance in diabetic rats and improve the degree of skeletal muscle lesion in T2DM rats by increasing the expression of GLUT4 protein in skeletal muscle,and promote the recovery of injured muscle.
