治未病理论及“三辨”模式下的胃息肉全程管理探析

摘　　要：结合治未病理论及辨体-辨病-辨证的“三辨”诊疗模式,对临床各阶段胃息肉患者实施个体化管理。微小息肉在针对偏颇体质用药的基础上加用行气开瘀、化湿燥痰之品,同时结合影像学特征分类管理,辅助辨证;胃息肉切除术后溃疡、腹痛、便溏等常见并发症以辨病-辨证为主结合辨体论治,以芍药甘草汤、佛手慈菇柴术方、养胃颗粒等加减应用,减轻症状;术后调养期的患者以纠偏体质为主、辨证指导遣方用药,从而减少胃息肉的再发。全程对患者给予饮食方式指导,以期达到未变先防、截断扭转、病后防复的目的。Individualized management of gastric polyps at different stages has been proposed based on the theory of preventive treatment of disease and the constitution-disease-syndrome differentiation model.On the basis of the drugs for the biased constitution,the products for promoting qi to open blood stasis and removing dampness and dryness phlegm were added,and the classification and management of the imaging characteristics were combined to assist the syndrome differentiation.The common complications such as ulcer,abdominal pain and loose stool after gastric polypectomy were treated mainly by disease differentiation and syndrome differentiation combined with body differentiation.Shaoyao Gancao Decoction,Foshou Cigu Chaizhu Formula and Yangwei Granules were used to add and subtract application to reduce the symptoms.In the postoperative convalescence period,patients should take corrective constitution as the main method and guide prescription medication based on syndrome differentiation,so as to reduce the recurrence of gastric polyps.In order to achieve the purpose of prevention before change,truncation and torsion,prevention and recovery after disease,patients were given dietary guidance throughout the whole process.
