摘　　要：原发性免疫球蛋白A肾病(IgAN)作为一种自身免疫病,病性虚实夹杂,病程长、易反复、难治愈,符合膜原病的特征。文章从膜原理论角度,认为膜原卫外不固是IgAN的发病基础,湿热困阻膜原是其关键环节,瘀毒阻滞膜原是疾病的进展原因,因此治疗IgAN以开达膜原为治则。具体分为补肾健脾以固膜原、芳香辛凉以透膜原、清利湿热以分膜原、散瘀解毒以通膜原。文章基于膜原理论诊治IgAN探究临床诊疗新思路,以期丰富膜原理论的科学内涵,为中医药治疗IgAN提供理论依据。Immune globulin A nephropathy(IgAN),as an autoimmune disease,is characterized by a mixture of deficiency and excess,a long course of disease,easy to repeat,and difficult to cure.From the perspective of membrane theory,this paper believes that the pathogenesis of IgAN is based on the unstability of the membrane,the blocking of the membrane by damp and heat is the key link,and the blocking of the membrane by stasis and toxin is the cause of disease progression.Therefore,the treatment of IgAN should be based on open membrane.Specifically,it can be divided into invigorating the kidney and strengthening the spleen to strengthen the membrane,fragrant,spicy and cool to penetrate the membrane,clearing away dampness and heat to divide the membrane,removing blood stasis and detoxicating to clear the membrane.Based on the theory of membrane,this paper explores new ideas of clinical diagnosis and treatment of IgAN,with a view to enriching the scientific connotation of membrane theory and providing theoretical basis for the treatment of IgAN with Chinese medicine.
