红黄煎剂改善乳腺癌蒽环类药物心脏毒性的随机对照试验

摘　　要：目的:评估红黄煎剂对乳腺癌蒽环类药物化疗早期心肌损伤的改善作用。方法:采用随机对照研究,将50例乳腺癌蒽环类药物化疗的患者平均分为治疗组和对照组。对照组行常规化疗,治疗组在每次化疗前1天至化疗结束后7天,服用红黄煎剂,每日2次,每次200 mL。观察患者化疗前及第1次化疗后3、6个月超声心动图斑点追踪成像技术左心室整体纵向应变(GLS)及左心室射血分数(LVEF),心肌酶,氧化应激指标。结果:化疗后6个月与化疗前比较,两组患者GLS值均显著下降(P<0.01),且治疗组显著高于对照组(P<0.05);治疗组IL-6显著下降(P<0.01),IL-10显著上升(P<0.01)。结论:对于早期乳腺癌使用蒽环类药物的患者,化疗期口服红黄煎剂有助于改善其早期心功能损伤,这种损伤通过GLS更能反映出来。Objective:To evaluate the effects of Chinese medicine Honghuang Decoction on early myocardial injury induced by anthracycline chemotherapy in breast cancer.Methods:A randomized controlled clinical trial was conducted.50 patients with breast cancer anthracycline chemotherapy were divided into the treatment group and the control group.The control group was conducted with coventional chemotherapy.The treatment group was treated with Honghuang Decoction twice a day,200 mL per time from the first day of chemotherapy to 7 days after chemotherapy.The left ventricular global longitudinal peak strain(GLS),left ventricular ejection fraction(LVEF),myocardial enzymes,and indicators of oxidative stress were measured before chemotherapy,3 months after the first time chemotherapy and 6 months after the first time chemotherapy.Results:After 6 months of chemotherapy compared with before chemotherapy,the GLS values of the two groups were significantly lower(P<0.01),and the GLS values of the treatment group were significantly higher than those of the control group(P<0.05).In the treatment group,IL-6 decreased significantly(P<0.01).Conclusion:For patients with early breast cancer treated with anthracycline,oral administration of Honghuang Decoction during chemotherapy may improve early cardiac function impairment,which is more reflected by GLS.
