乌蔹莓凝胶贴膏剂促进肛瘘术后创面愈合的临床疗效观察

摘　　要：目的:观察乌蔹莓凝胶贴膏剂促进肛瘘创面愈合的临床疗效。方法:将2020年11月—2021年6月南京中医药大学附属南京中医院所收治且行手术治疗的肛瘘患者72例,按随机数表法分为试验组和对照组,每组36例,试验组最终纳入30例,对照组最终纳入35例。试验组患者术后换药使用乌蔹莓凝胶贴膏剂,对照组患者术后换药使用无菌凡士林纱布,比较两组患者术后3、7、14 d创面面积、创面肉芽生长情况、疼痛指数及各组的不良反应。结果:治疗后,两组患者术后创面均于6月内完全愈合且试验组未见复发,对照组2例复发。术后7、14 d,试验组患者创面面积显著小于同期对照组(P<0.05,P<0.01),创面肉芽生长情况评分显著低于同期对照组(P<0.05)。术后3、7、14 d,试验组的视觉模拟评分均明显小于对照组(P<0.01,P<0.05)。试验组未见不良反应,对照组出现2例不良反应。结论:由乌蔹莓凝胶贴膏剂能够有效促进肛瘘术后创面的愈合,减轻患者疼痛,有较好的临床疗效,且使用过程中未出现不良反应事件,有较高的安全性。Objective:To observe the clinical efficacy of Causonis Japonica Herbe Cataplasms on wound healing of anal fistula.Methods:A total of 72 patients with anal fistula treated by surgery in Nanjing Hospital of Chinese Medicine,Nanjing University of Chinese Medicine from November 2020 to June 2021 were divided into a treatment group and a control group by random number table method,with 36 cases in each group.Ultimately,the treatment group included 30 cases and the control group included 35 cases.The treatment group used Causonis Japonica Herbe Cataplasms after surgery,and the control group used sterile Vaseline gauze after surgery.The wound area,wound granulation growth,pain index and adverse reactions of the two groups were compared 3,7 and 14 days after surgery.Results:After treatment,the wounds of both groups healed completely within 6 months and there was no recurrence in the treatment group and 2 cases of recurrence in the control group.The wound area of the treatment group was significantly smaller than that of the control group at 7 and 14 days after surgery(P<0.05,P<0.01).The growth of wound granulation in treatment group was significantly lower than that in control group(P<0.05).At 3,7,14 days after operation,the scores of visual analogue scale of the treatment group were significantly lower than that of the control group(P<0.01,P<0.05).There were no adverse reactions in the treatment group and 2 adverse reactions in the control group.Conclusion:Causonis Japonica Herbe Cataplasms can effectively promote wound healing after anal fistula operation,reduce the pain of patients,and has a good clinical effect,and there are no adverse events during the use with high safety.
