微通道经皮肾镜取石术与输尿管软镜碎石术治疗1.5 cm以下肾下盏结石的临床效果比较

摘　　要：目的比较微通道经皮肾镜取石术(MPCNL)与输尿管软镜碎石术(RIRS)治疗1.5 cm以下肾下盏结石的临床效果。方法选取162例1.5 cm以下肾下盏结石患者根据手术方式不同分为MPCNL组(n=79)和RIRS组(n=83),比较两组的围手术期指标、疼痛程度及并发症发生情况。结果两组的结石清除率、手术时间比较无统计学差异(P>0.05);RIRS组术中出血量低于MPCNL组,肠道功能恢复时间、住院时间短于MPCNL组(P<0.05)。RIRS组术后8 h、36 h的VAS评分低于MPCNL组(P<0.05)。两组术后并发症发生率比较无统计学差异(P>0.05)。结论MPCNL与RIRS治疗1.5 cm以下肾下盏结石的结石清除率与安全性相当;但与MPCNL相比,RIRS能显著减少患者术中出血量,减轻术后疼痛感,缩短术后恢复时间。Objective To compare the clinical effects of microchannel percutaneous nephrolithotomy(MPCNL)and retrograde intrarenal surgery(RIRS)in the treatment of lower renal calyx calculi below 1.5 cm.Methods 162 patients with lower renal calyx calculi below 1.5 cm were selected and divided into MPCNL group(n=79)and RIRS group(n=83)according to different surgical methods.The perioperative indicators,pain degree and complications were compared between the two groups.Results No statistical difference was found in stone clearance rate and operation time between the two groups(P>0.05).The intraoperative blood loss of the RIRS group was lower than that of the MPCNL group,and the recovery time of intestinal function and hospitalization time were shorter than those of the MPCNL group(P<0.05).8 hours and 36 hours after operation,the VAS scores of the RIRS group were lower than those of the MPCNL group(P<0.05).No statistical difference was found in the incidence of postoperative complications between the two groups(P>0.05).Conclusions Stone removal rate and safety of MPCNL and RIRS in the treatment of lower renal calyx calculi below 1.5 cm are comparable;however,compared with MPCNL,RIRS can significantly reduce the intraoperative blood loss of patients,alleviate the postoperative pain,and shorten the postoperative recovery time.
