摘 要:目的:探讨多普勒超声在鉴别诊断小儿先天性心脏病类型中的价值。方法:选取2021年5月—2022年12月收治的疑似先天性心脏病患儿67例,均行多普勒超声、心血管造影检查,依据心血管造影检查结果,分析多普勒超声对小儿先天性心脏病的诊断及鉴别诊断价值。结果:67例疑似小儿先天性心脏病患儿经心血管造影确诊56例(83.58%)。其中动脉管道未闭6例,法洛四联症9例,室间隔缺损23例,心内膜垫缺损3例,房室间隔缺损7例,三尖瓣下畸形8例。依据心血管造影检查结果,多普勒超声对小儿先天性心脏病诊断的灵敏度为100.00%,特异度为81.82%,准确度为97.01%,且与心血管造影检查结果一致性好(Kappa值=0.883)。依据心血管造影检查结果,多普勒超声对小儿先天性心脏病类型鉴别准确度为92.86%,且与心血管造影检查结果一致性好(Kappa值=0.906)。结论:多普勒超声对小儿先天性心脏病有较高诊断价值,且可有效鉴别小儿先天性心脏病类型。Objective:To explore the value of doppler ultrasonography in differential diagnosis of congenital heart disease in children.Methods:All sity-seven suspected children with congenital heart disease admitted to the hospital from May 2021 to December 2022 were selected to undergo doppler ultrasound and angiocardiography.According to the results of angiocardiography,the value of Doppler ultrasound in diagnosis and type differentiation of congenital heart disease in children was analyzed.Results:In 67 suspected children with congenital heart disease,56 cases(83.58%)were confirmed by angiography,including 6 cases of patent arterial duct,9 cases of tetralogy of fallot,23 cases of ventricular septal defect,3 cases of endocardial pad defect,7 cases of atrioventricular septal defect and 8 cases of subtricuspid valve malformation.According to the results of angiocardiography,the sensitivity,specificity and accuracy of doppler ultrasound in the diagnosis of congenital heart disease in children were 100.00%,81.82%and 97.01%,which were in good agreement with the results of angiocardiography(Kappa value=0.883).According to the results of CVD examination,the accuracy of doppler ultrasound in distinguishing types of congenital heart disease in children was 92.86%,and it was in good agreement with the results of CVD examination(Kappa value=0.906).Conclusion:Doppler ultrasound has high diagnostic value in children with congenital heart disease,and can effectively identify the types of congenital heart disease in children.
