良性阵发性位置性眩晕患者的耳石器及半规管功能分析

摘　　要：目的探讨良性阵发性位置性眩晕(BPPV)对耳石器及半规管功能的影响及其可能的发病因素。方法选择2022年12月-2023年6月本院收治的单管良性阵发性位置性眩晕患者作为研究对象,分别行视频头脉冲试验(vHIT)、前庭肌源诱发电位(VEMP)、听性脑干反应(ABR),分析其检测结果。结果后半规管BPPV患者与水平半规管BPPV患者vHIT、cVEMP、oVEMP、ABR检查结果异常侧别分布情况比较,差异无统计学意义(P>0.05),但在老年水平半规管BPPV患者的oVEMP双侧异常率(41.2%)比后半规管BPPV患者的oVEMP双侧异常率(10.3%)高,两组差异存在统计学意义(P<0.05)。青中年BPPV患者的ABR双侧异常率(15.2%)低于老年BPPV患者的ABR双侧异常率(33.9%),差异存在统计学意义(P<0.05);青中年BPPV患者与老年BPPV患者vHIT、cVEMP、oVEMP检查结果异常侧别分布情况比较,差异无统计学意义(P>0.05)。结论BPPV可见于半规管弥漫性损伤,内耳循环障碍可引起耳石器功能障碍,其可能是BPPV的发病因素。Objective To investigate the impact of benign paroxysmal positional vertigo(BPPV)on otolith and semicircular canals and its possible pathogenesis factors.Methods Patients with single-tube benign paroxysmal positional vertigo who were treated in our hospital from December 2022 to June 2023 were enrolled as study subjects.Video head impulse test(vHIT),vestibular myogenic evoked potential(VEMP),and auditory brainstem response(ABR)were conducted on them,and the results of examination were analyzed.Results There was no significant difference in the abnormal lateral distribution of vHIT,cVEMP,oVEMP,and ABR between the posterior semicircular canal BPPV patients and the horizontal semicircular canal BPPV patients(P>0.05).However,the bilateral oVEMP abnormality rate of elderly horizontal semicircular canal BPPV patients(41.2%)was higher than that of posterior semicircular canal BPPV patients(10.3%),and the difference between the two groups was statistically significant(P<0.05).The bilateral ABR abnormality rate in young and middle-aged BPPV patients(15.2%)was lower than that in elderly BPPV patients(33.9%),and the difference was statistically significant(P<0.05).There was no significant difference in the distribution of abnormal sides of vHIT,cVEMP,and oVEMP between young and middle-aged BPPV patients and elderly BPPV patients(P>0.05).Conclusions BPPV can be associated with diffuse injury to the semicircular canals.Inner ear circulation disturbances may lead to otolith dysfunction,which could be a pathogenic factor of BPPV.
