双歧杆菌联合蓝光照射治疗新生儿感染性黄疸的效果及对S100B、Aβ及网织红细胞参数水平比较

摘  要:目的观察双歧杆菌联合蓝光照射治疗新生儿感染性黄疸的效果,分析其对S100B蛋白(S100B)、β淀粉样蛋白(Aβ)及网织红细胞参数水平的影响。方法选择2020年11月-2022年10月在如皋市人民医院接受治疗的95例新生儿感染性黄疸患儿,采用随机数表法分为试验组(n=48)和对照组(n=47)。对照组给予蓝光照射治疗,试验组给予双歧杆菌联合蓝光照射治疗。比较两组临床疗效、血清S100B、Aβ、低核酸网织红细胞比例(L-RTC)、高荧光网织红细胞比例(H-TC)、网织红细胞(RET)、光疗时间、胎便排空时间、退黄时间、住院时间及不良反应发生情况。结果治疗后,试验组总有效率为87.50%(42/48),明显高于对照组的70.21%(33/47),差异有统计学意义(χ^(2)=4.270,P<0.05)。治疗后,两组血清S100B、Aβ、L-RTC、RET水平均降低,H-TC水平升高,且试验组较对照组降低或升高更为明显,差异均有统计学意义(t=6.424、22.603、2.962、3.410、2.613,P均<0.05)。试验组患儿光疗时间、胎便排空时间、退黄时间及住院时间均低于对照组,差异均有统计学意义(t=6.593、6.557、3.510、4.604,P均<0.05)。治疗期间,试验组患儿皮疹、腹泻、发热、血钙下降等不良反应发生率为8.33%(4/48),低于对照组的23.40%(11/47),差异有统计学意义(χ^(2)=4.057,P<0.05)。结论双歧杆菌联合蓝光照射治疗新生儿感染性黄疸具有较好的疗效,可能与其可改善S100B、Aβ及网织红细胞参数水平有关。Objective To study the effects of Bifidobacterium combined with blue light irradiation on neonatal jaundice caused by infection and the levels of S100 calcium binding protein B(S100B),amyloidβprotein(Aβ)and reticulocytic parameters.Methods A total of 95 newborns with neonatal jaundice caused by infection who received treatment in the Rugao People's Hospital from November 2020 to October 2022 were randomly divided into an experimental group(n=48)and a control group(n=47)using a random number table method.The control group was treated with blue light irradiation,while the experimental group was treated with bifidobacteria combined with blue light irradiation.The clinical efficacy was compared,in terms of serum S100B,Aβ,low nucleic acid reticulocyte ratio(L-RTC),high fluorescent reticulocyte(H-TC),reticulocyte(RET),phototherapy time,defecation emptying time,jaundice removal time,hospitalization time,and incidence of adverse reactions.Results After treatment,the total response rate in the experimental group was 87.50%(42/48),which was significantly higher than the 70.21%(33/47)in the control group(χ^(2)=4.270,P<0.05).After treatment,the serum levels of S100B,Aβ,L-RTC and H-TC were significantly decreased,and the RET levels significantly increased(t=6.424,22.603,2.962,3.410,2.613;all P<0.05).The time of light therapy,the time of fetal defecation,the time of yellowing and the time of hospitalization in the experimental group were significantly lower than those in the control group(t=6.593,6.557,3.510,4.604;all P<0.05).During the treatment period,the incidence of adverse reactions such as rash,diarrhea,fever,and blood calcium decline in the experimental group was 8.33%(4/48),lower than 23.40%(11/47)of the control group,and the difference was significant(χ^(2)=4.057,P<0.05).Conclusion Bifidobacterium combined with blue light irradiation had a good effect on neonatal jaundice caused by infection,which might improve S100B,Aβand reticulocyte parameter levels.
