摘  要:目的探究miR-21和miR-17-5p检测对慢性心力衰竭(CHF)诊断效能。方法选择2022年1月至2023年11月于三亚中心医院收治的68例CHF患者(CHF组)的临床资料进行回顾性研究,另选取同期我院健康体检者20例作为对照组。比较两组患者miR-21、miR-17-5p、心功能指标[左心房(LA)、左心室(LV)、右心房(RA)、右心室(RV)、射血分数(EF)、室间隔(IVS)]、血清指标[总胆固醇(TC)、三酰甘油(TG)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、白蛋白(ALB)、尿素氮(BUN)、肌酐(CR)、尿酸(UA)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、白细胞(WBC)、血红蛋白(HGB)、血小板计数(PLT)]的差异,通过ROC曲线分析miR-21和miR-17-5p诊断慢性心力衰竭的价值。结果CHF组miR-21、miR-17-5p均高于对照组(P<0.05)。两组LA、LV、RA、RV、EF比较,差异均无统计学意义(P>0.05),但CHF组IVS高于对照组(P<0.05)。两组TC、TG、LDL-C、HDL-C、WBC、PLT、ALT、AST、ALB、BUN、CR、UA比较,差异均无统计学意义(P>0.05),CHF组HGB高于对照组(P<0.05)。Pearson相关性分析显示,miR-21和miR-17-5p呈正相关(P<0.05)。ROC曲线分析显示,miR-21、miR-17-5p诊断CHF的AUC分别为0.877、0.861;敏感度分别为77.1%、90.0%;特异度分别为70.8%、90.0%。将miR-21、mi R-17-5p纳入二元Logistic回归模型,联合数据诊断CHF的AUC为0.884;敏感度为79.2%;特异度为90.0%。结论Mi R-21、mi R-17-5p在CHF血清中高表达,可作为诊断的有效指标,且两者存在正相关。Objective To discuss the efficacy of detections of microRNA-21(miR-21)and microRNA-17-5p(miR-17-5p)in heart failure diagnosis.Methods The patients with chronic heart failure(CHF,n=68)were chosen into CHF group and healthy controls(n=20)were chosen into control group from Sanya Central Hospital from Jan.2022 to Nov.2023,and their clinical materials were retrospectively studied.The levels of miR-21 and miR-17-5p,heart function indexes[left atrium(LA),left ventricle(LV),right atrium(RA),right ventricle(RV),ejection fraction(EF),interventricular septum(IVS)],and serum indexes[total cholesterol(TC),triglycerides(TG),alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin(ALB),blood urea nitrogen(BUN),creatinine(CR),uric acid(UA),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),white blood cell(WBC),hemoglobin(HGB),platelet(PLT)]were compared between 2 groups.The diagnostic value of miR-21 and miR-17-5p to CHF was analyzed by using ROC curve.Results The levels of miR-21 and miR-17-5p were higher in CHF group than those in control group(P<0.05).The differences in LA,LV,RA,RV and EF had no statistical significance between 2 groups(P>0.05),while IVS was higher in CHF group than that in control group(P<0.05).The differences in TC,TG,LDL-C,HDL-C,WBC,PLT,ALT,AST,ALB,BUN,CR and UA had no statistical significance between 2 groups(P>0.05),while HGB was higher in CHF group than that in control group(P<0.05).The results of Pearson correlation analysis showed that miR-21 was positively correlated to miR-17-5p(P<0.05).The results of ROC curve analysis showed that,in CHF diagnosis,AUC of miR-21 was 0.877,sensitivity was 77.1%and specificity was 70.8%,and AUC of miR-17-5p was 0.861,sensitivity was 90.0%and specificity was 90.0%.When miR-21 and miR-17-5p were incorporated into binary Logistic regression model,AUC of combined miR-21 and miR-17-5p in CHF diagnosis was 0.884,sensitivity was 79.2%and specificity was 90.0%.Conclusion The high expressions of serum miR-21 and miR-17-5p can
