摘　　要：目的分析急性冠状动脉综合征患者血栓弹力图(thrombelastograghy,TEG)相关影响因素。方法回顾性分析2018年4—11月北京大学国际医院收治的66例急性冠状动脉综合征患者的临床资料,所有患者均接受TEG检测。66例患者根据花生四烯酸(arachidonic acid,AA)途径血小板抑制率分为A组与B组;55例加用氯吡格雷患者根据二磷酸腺苷(adenosine diphosphate,ADP)途径血小板抑制率分为C组与D组,根据ADP的MA值分为E组与F组,根据总MA值分为G组与H组,根据R时间分为I组与J组,根据K时间分为K组与L组,根据α角分为M组与N组;28例进行2次TEG检测的患者,以第1次TEG检测的R时间作为O组,第2次TEG检测的R时间作为P组。比较A组与B组、C组与D组、E组与F组、G组与H组、I组与J组、K组与L组、M组与N组年龄、性别、体质量、吸烟史、高血压、糖尿病、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)水平,比较O组与P组R时间。结果A组与B组、C组与D组、E组与F组、G组与H组、I组与J组、K组与L组、M组与N组的年龄、性别、体质量、吸烟史、高血压、糖尿病、LDL-C水平比较差异无统计学意义。O组R时间短于P组,差异有统计学意义(P<0.05)。结论AA途径血小板抑制率、ADP途径血小板抑制率、ADP的MA值、总MA值、R时间、K时间、α角与年龄、性别、体质量、吸烟史、高血压、糖尿病、LDL-C水平无关。Objective To analyze the related factors of thromboelastography(TEG)in patients with acute coronary syndrome.Methods The clinical data of 66 patients with acute coronary syndrome at Peking University International Hospital from April to November 2018 were retrospective analyzed,all patients underwent TEG testing.66 patients were divided into the group A and the group B based on the platelet inhibition rate through the arachidonic acid(AA)pathway;55 patients who received clopidogrel were divided into the group C and the group D based on the ADP pathway platelet inhibition rate,the group E and the group F based on the ADP MA value,the group G and the group H based on the total MA value,the group I and the group J based on the R time,the group K and the group L based on the K time,and the group M and the group N based on theαangle;28 patients underwent two TEG tests,with the R time of the first TEG test as the group O and the R time of the second TEG test as the group P.The age,sex,weight,smoking history,hypertension,diabetes,low density lipoprotein cholesterol(LDL-C)levels of the group A and the group B,the group C and the group D,the group E and the group F,the group G and the group H,the group I and the group J,the group K and the group L,the group M and the group N were compared,and the R time of the group O and the group P were compared.Results There were no significant differences in age,sex,weight,smoking history,hypertension,diabetes and LDL-C levels between the group A and the group B,the group C and the group D,the group E and the group F,the group G and the group H,the group I and the group J,the group K and the group L,and the group M and the group N.The R time in the group O was shorter than that in the group P,and the differences were statistically significant(P<0.05).Conclusion The AA pathway platelet inhibition rate,ADP pathway platelet inhibition rate,ADP MA value,total MA value,R time,K time andαangle were not related to age,sex,weight,smoking history,hypertension,diabetes and LDL-C levels.
