摘  要:目的探讨输血检验中凝聚胺法与微柱凝胶法的应用效果及检验结果。方法选取2022年1月至2023年6月赣州市人民医院收治的116例临床输血患者作为研究对象,均采用凝聚胺法与微柱凝胶法进行不完全抗体筛查,比较两种筛查结果,包括交叉配血检测时间、交叉配血结果、不完全抗体检测灵敏度和特异度。结果聚胺法交叉配血检测时间为(10.94±2.66)min,短于微柱凝胶法的(28.54±2.74)min,差异有统计学意义(P<0.05)。两种方法交叉配血结果中的主侧阳性率、次侧阳性率、假阳性率比较差异无统计学意义;凝聚胺法总阳性率为4.31%,低于微柱凝胶法的12.07%,差异有统计学意义(P<0.05)。微柱凝胶法筛查出不完全抗体患者14例,其中抗C 1例(0.86%)、抗D 4例(3.45%)、抗E 8例(6.90%)、抗e 1例(0.86%);凝聚胺法筛查出不完全抗体患者5例,其中抗C 1例(0.86%)、抗D 1例(0.86%)、抗E 2例(1.72%)、抗e 1例(0.86%)。微柱凝胶法对不完全抗体检测的灵敏度、特异度、正确诊断指数及符合率分别为96.28%、99.42%、0.71、99.37%,凝聚胺法对不完全抗体检测的灵敏度、特异度、正确诊断指数及符合率分别为94.35%、99.14%、0.63、99.10%。结论微柱凝胶法检出阳性率、检测结果稳定性和不完全抗体检测的灵敏度更高,但凝聚胺法配血速度更快。Objective To explore the application effect and test results between polyamine method and microcolumn gel method in blood transfusion test.Methods A total of 116 clinical transfusion patients admitted to Ganzhou People's Hospital from January 2022 to June 2023 were selected as the study subjects,incomplete antibody screening was performed by the coagglutamine method and the microcolumn gel method.The results of the two screening methods were compared,including the detection time of cross matching blood,the results of cross matching blood,the sensitivity and specificity of incomplete antibody detection.Results The detection time of polyamine cross-matching blood was(10.94±2.66)min,which was shorter than(28.54±2.74)min of microcolumn gel method,and the difference was statistically significant(P<0.05).There were no significant differences in the main positive rate,secondary positive rate and false positive rate between the two methods,the total positive rate of coacylamine method was 4.31%,which was lower than 12.07%of microcolumn gel method,the difference was statistically significant(P<0.05).Fourteen patients with incomplete antibodies were screened by microcolumn gel method,including 1(0.86%)anti-C,4(3.45%)anti-D,8(6.90%)anti-E and 1(0.86%)anti-E.Five patients with incomplete antibodies were screened by agglutamine method,including 1(0.86%)anti-C,1(0.86%)anti-D,2(1.72%)anti-E and 1(0.86%)anti-E.The sensitivity,specificity,correct diagnosis index and coincidence rate of incomplete antibody detection by microcolumn gel method were 96.28%,99.42%,0.71 and 99.37%respectively.The sensitivity,specificity,correct diagnosis index and coincidence rate of the method were 94.35%,99.14%,0.63 and 99.10%,respectively.Conclusion The positive rate,stability and sensitivity of incomplete antibody detection by microcolumn gel method are higher,but the speed of blood matching by polyamine method is faster.
