摘  要:目的探究不同时机应用枸橼酸咖啡因对早产儿呼吸暂停防治的临床价值。方法选取2019年1月至2022年1月于吉安市妇幼保健院出生的70例早产儿作为研究对象,按照用药时机不同分为A组(发生呼吸暂停时开始应用枸橼酸咖啡因)与B组(出生后2 h应用枸橼酸咖啡因),各35例。比较两组用药24 h后呼吸暂停发生率、并发症发生率、死亡率、康复时间及不良反应发生率。结果B组用药24 h后呼吸暂停及脑室内出血、早产儿视网膜病、支气管发育不良发生率均低于A组,差异有统计学意义(P<0.05);两组死亡率比较差异无统计学意义。B组枸橼酸咖啡因治疗时间、氧气支持时间、机械通气时间、全胃肠喂养时间、住院时间均短于A组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论与发生呼吸暂停时应用枸橼酸咖啡因治疗相比,早产儿出生后2 h使用枸橼酸咖啡因治疗可降低用药24 h后呼吸暂停发生率及相关并发症发生率,缩短康复时间,未增加药物不良反应发生。Objective To explore the clinical value of caffeine citrate on apnea in premature infants at different times.Methods A total of 70 premature infants born in the Ji'an Maternal and Child Health Hospital from January 2019 to January 2022 were selected as the research subjects,they were divided into the group A(caffeine citrate was used when apnea occurred)and the group B(caffeine citrate was used at 2 h after birth)according to the timing of medication,with 35 cases in each group.The incidence of apnea after 24 h of medication,complications,mortality,recovery time and adverse reactions were compared between the two groups.Results The incidence of apnea after 24 h of medication,intraventricular hemorrhage,retinopathy of prematurity and bronchial dysplasia in the group B were lower than those in the group A,and the differences were statistically significant(P<0.05);there was no significant difference in mortality between the two groups.The treatment time of caffeine citrate,oxygen support time,mechanical ventilation time,total gastrointestinal feeding time and hospitalization time in the group B were shorter than those in the group A,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Compared with the use of caffeine citrate when apnea occurs,the use of caffeine citrate 2 h after birth in premature infants can reduce the incidence of apnea and related complications 24 h after medication,shorten the recovery time,and do not increase the incidence of adverse drug reactions.
