地诺前列酮栓在足月胎膜早破低宫颈评分初产妇引产中的应用效果

摘　　要：目的 探讨地诺前列酮栓在足月胎膜早破低宫颈评分初产妇引产中的应用效果。方法 选取2023年2月至2024年2月聊城市东昌府区妇幼保健院收治的88例足月胎膜早破初产妇作为研究对象,根据抽签法分为两组,各44例。对照组静脉滴注缩宫素引产,治疗组给予地诺前列酮栓引产。比较两组促宫颈成熟效果、用药到规律宫缩时间及总产程、分娩方式、妊娠并发症及不良妊娠结局发生情况、新生儿结局、不良反应发生情况。结果 治疗组促宫颈成熟的总有效率高于对照组(P<0.05)。治疗组用药到规律宫缩时间及总产程均短于对照组(P<0.05)。治疗组剖宫产率低于对照组(P<0.05)。治疗组妊娠并发症及不良妊娠结局总发生率低于对照组(P<0.05)。两组新生儿结局及不良反应总发生率比较,差异无统计学意义(P> 0.05)。结论 足月胎膜早破初产妇采用地诺前列酮栓的促宫颈成熟效果更好,可缩短产程,降低剖宫产率及妊娠并发症、不良妊娠结局发生率。Objective To explore the application effect of Dinoprostone Suppositories in induction of labor of primipara with term premature rupture of membranes and low cervical score.Methods A total of 88 primiparas with term premature rupture of membranes who were admitted to Maternal and Child Health Hospital of Liaocheng Dongchangfu District from February 2023 to February 2024 were selected as the study objects.All the primiparas were divided into two groups by the lottery method,with 44 cases in each group.The control group received intravenous infusion of Oxytocin for induction of labor,while the treatment group received Dinoprostone Suppositories for induction of labor.The effect on promoting cervical ripening,using medication until regular uterine contraction time,total stage of labor,delivery mode,incidence of pregnancy complications and adverse pregnancy outcomes,neonatal outcomes,and incidence of adverse reactions between the two groups were compared.Results The total effective rate of promoting cervical ripening in the treatment group was higher than that in the control group(P<0.05).The using medication until regular uterine contraction time and total stage of labor in the treatment group were shorter than those in the control group(P<0.05).The cesarean section rate in the treatment group was lower than that in the control group(P<0.05).The total incidence of pregnancy complications and adverse pregnancy outcomes in the treatment group was lower than that in the control group(P<0.05).There were no statistically significant differences in the neonatal outcomes and the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The use of Dinoprostone Suppositories to promote cervical ripening in primipara with term premature rupture of membranes is good,which can shorten labor,reduce cesarean section rate and the incidence of pregnancy complications and adverse pregnancy outcomes.
