摘  要:目的:探讨Ⅱ/Ⅲ型迟发性创伤后缺血性椎体骨坏死病(Kümmell's病)应用骨水泥强化椎弓根螺钉锚定技术治疗的临床疗效。方法:回顾性分析32例Ⅱ/Ⅲ型迟发性创伤后缺血性椎体骨坏死病患者资料,其中男12例,女20例;年龄为61~85岁,病程为3~12个月。所有患者均采用骨水泥强化椎弓根螺钉锚定技术进行治疗,评估手术持续时间、术中出血量及并发症发生率;比较术前及术后不同时间点视觉模拟量表(VAS)评分、Oswestry功能障碍指数(ODI)及后凸Cobb角。结果:所有患者均获随访至术后3个月。手术时间为(42.8±13.4)min,术中出血量为(52.5±5.4)mL。术后未出现感染及骨水泥松动病例。术后各时间点VAS评分及ODI评分低于术前,差异有统计学意义(P<0.05);Cobb角小于术前,差异有统计学意义(P<0.05)。患者疼痛明显改善,生活质量明显提高。结论:骨水泥强化椎弓根螺钉锚定技术治疗Ⅱ/Ⅲ型Kümmell's病简便有效,并能降低术后并发症,值得临床推广应用。Objective:To investigate the clinical effect of bone cement-reinforced pedicle screw anchoring technique in the treatment of typeⅡ/Ⅲdelayed post-traumatic ischemic vertebral osteonecrosis(Kümmell's disease).Methods:A total of 32 patients with typeⅡ/ⅢKümmell's disease were retrospectively analyzed,including 12 males and 20 females.The age ranged from 61 to 85 years.The disease duration ranged from 3 to 12 months.All patients with bone cement to strengthen pedicle screw anchoring technique treatment,duration of surgery,intraoperative blood loss,and complications.The visual analogue scale(VAS),Oswestry disability index(ODI)and kyphosis Cobb angle were compared before and after operation.Results:All patients were followed up to 3 months after operation.The operation time was(42.8±13.4)min,the intraoperative blood loss was(52.5±5.4)mL.No infection or loosening of bone cement occurred after operation.After each time point of VAS score,ODI score is lower than the preoperative(P<0.05),Cobb angle is less than the preoperative(P<0.05).The pain was significantly improved and the quality of life was significantly improved.Conclusion:The bone cement to strengthen the pedicle screw anchoring technology treatmentⅡ/Ⅲtype Kümmell's disease has a simple and effective,and can effectively reduce the postoperative complications,worthy of further popularization and application in clinic.
