醋酸去氨加压素联合缩泉丸治疗儿童遗尿症的疗效及对睡眠质量的影响

摘　　要：目的 探讨醋酸去氨加压素联合缩泉丸治疗儿童遗尿症的疗效及对睡眠质量的影响。方法 选取2021年1月~2023年12月在某院就诊的75例遗尿症患儿进行研究。使用随机数字表法将75例患儿随机分为对照组(n=38,醋酸去氨加压素)与观察组(n=37,醋酸去氨加压素联合缩泉丸)。比较两组患儿的临床疗效、中医症候积分、临床指标(平均每周夜晚遗尿次数、单次尿量、夜间日间尿量比值)、实验室指标[尿渗透压、尿血渗透压比、抗利尿激素(ADH)]、睡眠质量[匹兹堡睡眠质量指数量表(PSQI)]以及药物相关不良反应。结果 观察组总有效率为94.59%,高于对照组的78.95%(P <0.05)。治疗2个疗程末,观察组尿频且少、尿液浑浊、情志抑郁、食欲不振积分均较对照组低(P <0.05)。治疗期间观察组平均每周夜晚遗尿次数、单次尿量少于对照组,夜间日间尿量比值低于对照组(P<0.05)。治疗2个疗程末,观察组尿渗透压、尿血渗透压比、ADH均高于对照组(P <0.05),观察组PSQI评分(5.95±1.36)分低于对照组(7.50±1.54)分(P <0.05)。观察组不良反应发生率(10.81%)高于对照组(7.89%),但差异无统计学意义(P> 0.05)。结论 醋酸去氨加压素联合缩泉丸治疗儿童遗尿症疗效良好,可提高临床疗效,缓解临床症状,减少夜晚遗尿次数和单次尿量,改善实验室指标与睡眠质量,安全性良好。Objective To explore the curative effect of desmopressin acetate combined with Suoquan pill and its influences on sleep quality in children with enuresis.Methods A total of 75 children with enuresis in the hospital were enrolled between January 2021 and December 2023.According to random number table method,they were divided into the control group(n=38,desmopressin acetate)and the observation group(n=37,desmopressin acetate combined with Suoquan pills).The clinical curative effect,scores of TCM symptoms,clinical indexes(average times of weekly nighttime enuresis,single urine volume,ratio of nighttime/daytime urine volume),laboratory indexes[urine osmotic pressure,ratio of urine/blood osmotic pressure,antidiuretic hormone(ADH)],sleep quality[Pittsburgh Sleep Quality Index(PSQI)]and drug-related adverse reactions were compared between the two groups.Results The total response rate of observation group was higher than that of control group(94.59%vs.78.95%,P<0.05).After 2 courses of treatment,scores of TCM symptoms(frequent and little urination,turbid urine,emotional depression,loss of appetite)in observation group were lower than those in control group(P<0.05).During treatment,average times of weekly nighttime enuresis and single urine volume in observation group were lower than those in control group,and ratio of nighttime/daytime urine volume was lower than that in control group(P<0.05).After 2 courses of treatment,urine osmotic pressure,ratio of urine/blood osmotic pressure and ADH in observation group were higher than those in control group(P<0.05),and PSQI score was lower than that in control group[(5.95±1.36)vs.(7.50±1.54)](P<0.05).The incidence of adverse reactions in observation group was higher than that in control group(10.81%vs.7.89%),but the difference was not statistically significant(P>0.05).Conclusion The curative effect of desmopressin acetate combined with Suoquan pill is good in children with enuresis,which can improve clinical curative effect,relieve clinical symptoms,reduce times of nighttime en
