多模态磁共振成像对宫颈癌国际妇产科联盟分期和淋巴结转移的评估价值研究

摘　　要：目的分析宫颈癌国际妇产科联盟(FIGO)分期及淋巴结转移评估中多模态磁共振成像(MRI)的应用价值。方法选取2021年1月至2023年1月我院收治的52例宫颈癌患者,入组者均行多模态MRI检查,以手术病理诊断结果为金标准,分析多模态MRI检查结果。结果52例宫颈癌患者经手术活检显示,FIGO分期为Ⅰ期14例(26.9%)、Ⅱ期23例(44.2%),Ⅲ期9例(17.3%),Ⅳ期6例(11.5%);多模态MRI对FIGO分期的诊断结果为:Ⅰ期12例,Ⅱ期24例,Ⅲ期7例,Ⅳ期6例;多模态MRI对宫颈癌FIGO分期为Ⅰ期、Ⅱ期、Ⅲ期及Ⅳ期的诊断准确度分别为88.5%、90.4%、92.3%及100%;52例宫颈癌患者经手术检出淋巴结共148个,其中淋巴结转移71个;多模态MRI检查显示,52例宫颈癌患者共有淋巴结转移67个,诊断敏感度、特异度及准确度分别为94.4%(67/71)、90.9%(70/77)、93.2%(138/148)。结论宫颈癌诊断中多模态MRI的应用价值较高,可准确判断FIGO分期,并评估淋巴结转移,以指导临床诊治。Objective To analyze the application value of multimodal magnetic resonance imaging(MRI)in the assessment of FIGO staging and lymph node metastasis in cervical cancer.Methods Fifty-two cervical cancer patients admitted to our hospital from January 2021 to January 2023 were selected and enrolled and underwent multimodal MRI,and the results of multimodal MRI were analyzed with the results of surgical pathological diagnosis as the gold standard.Results The 52 cervical cancer patients underwent surgical biopsy,which showed that there were 14 cases(26.9%)of FIGO stageⅠ,23 cases(44.2%)of stageⅡ,9 cases(17.3%)of stageⅢ,and 6 cases(11.5%)of stageⅣ.The diagnostic results of multimodal MRI for the FIGO stage were:12 cases of stageⅠ,24 cases of stageⅡ,7 cases of stageⅢ,and 6 cases of stageⅣ.The diagnostic accuracy of multimodal MRI for FIGO staging of cervical cancer was 88.5%,90.4%,92.3%and 100%for stageⅠ,Ⅱ,ⅢandⅣ.A total of 148 lymph nodes were detected in 52 patients with cervical cancer by surgery,of which 71 lymph nodes were metastatic.The multimodal MRI examination showed a total of 67 lymph node metastasis in 52 patients with cervical cancer,and the diagnostic sensitivity,specificity and accuracy were 94.4%(67/71),90.9%(70/77)and 93.2%(138/148),respectively.Conclusion The application value of multimodal MRI in the diagnosis of cervical cancer is high,and it can accurately determine the FIGO stage and evaluate lymph node metastasis to guide clinical diagnosis and treatment.
