自拟健脾渗湿方加减联合胰岛素强化治疗2型糖尿病黎明现象患者的效果

摘　　要：目的:观察自拟健脾渗湿方加减联合胰岛素强化治疗2型糖尿病黎明现象患者的效果。方法:选取2022年2月至2023年3月该院收治的76例2型糖尿病黎明现象患者进行前瞻性研究,按照随机数字表法将其分为研究组和对照组各38例。对照组采用胰岛素强化治疗,研究组在对照组基础上联合自拟健脾渗湿方加减治疗,两组均连续治疗1个月。比较两组临床疗效,治疗前后中医证候积分、血糖指标[空腹血糖(FPG)、餐后2 h血糖(2hPG)、空腹血糖与夜间最低血糖的净增值(ΔGLU)]水平,以及不良反应发生率。结果:研究组治疗总有效率为92.11%(35/38),高于对照组的71.05%(27/38),差异有统计学意义(P<0.05);治疗后,研究组中医证候积分和FPG、2hPG、ΔGLU水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:自拟健脾渗湿方加减联合胰岛素强化治疗2型糖尿病黎明现象患者可提高治疗总有效率,降低中医证候积分和血糖指标水平,效果优于单纯胰岛素强化治疗。Objective:To observe effects of self-made Jianpi Shenshi decoction combined with intensive insulin therapy on patients with type 2 diabetes and dawn phenomenon.Methods:A prospective study was conducted on 76 patients with type 2 diabetes and dawn phenomenon admitted to this hospital from February 2022 to March 2023.According to the random number table method,they were divided into study group and control group,38 cases in each group.The control group was treated with intensive insulin therapy,while the study group was treated with self-made Jianpi Shenshi decoction on the basis of that of the control group.Both groups were treated continuously for 1 month.The clinical efficacy,the TCM syndrome scores,the blood glucose indexes[fasting blood glucose(FPG),2 h postprandial blood glucose(2hPG),fasting blood glucose and net increase of the lowest blood glucose at night(ΔGLU)]levels before and after the treatment,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the study group was 92.11%(35/38),which was higher than 71.05%(27/38)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the scores of TCM syndromes and the levels of FPG,2hPG andΔGLU in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Self-made Jianpi Shenshi decoction combined with insulin intensive treatment for the patients with type 2 diabetes and dawn phenomenon can improve the total effective rate of treatment and reduce the TCM syndrome scores and the blood glucose index levels.Moreover,it is superior to simple intensive insulin therapy.
