摘 要:目的:探讨不切开股外侧肌的前外侧入路(ALA)在髋关节置换术中的应用效果。方法:收集2020年6月-2021年12月于清远市清新区人民医院初次接受人工全髋关节置换术的68例患者的临床资料,按照不同手术方式将患者分为传统切开股外侧肌的ALA组(对照组,n=30)与不切开股外侧肌的ALA组(研究组,n=38)。比较两组手术相关指标、术后并发症、髋关节功能及假体匹配情况。结果:研究组术后第3天疼痛评分低于对照组,差异有统计学意义(P<0.05);两组住院时间、切口长度、手术时间、术中出血量、术后引流量、术后第1天疼痛评分、并发症发生率、假体匹配率比较,差异无统计学意义(P>0.05)。术后1周、1个月,两组Harris评分较术前提高,随时间增加Harris评分越高,且研究组高于对照组,差异有统计学意义(P<0.05)。结论:相比于切开股外侧肌的ALA,不切开股外侧肌更有利于减轻患者疼痛,利于患者更快康复。Objective:To investigate the application effect of the anterolateral approach(ALA)without the lateral femoris muscle incision in hip replacement.Method:The clinical data of 68 patients who underwent artificial total hip arthroplasty for the first time in People's Hospital of Qingxin District in Qingyuan City from June 2020 to December 2021 were collected,the patients were divided into ALA group with traditional lateral femoral muscle incision(the control group,n=30)and ALA group without lateral femoral muscle incision(the study group,n=38)according to different surgical methods.The operation related indexes,postoperative complications,hip function and prosthesis matching were compared between two groups.Result:The pain score of the study group on the third day after operation was lower than that of the control group,and the difference was statistically significant(P<0.05);there were no significant differences in the hospitalization time,incision length,operation time,intraoperative blood loss,postoperative drainage volume,pain score on the first day after operation,complication rate and prosthesis matching rate between two groups(P>0.05);at 1 week and 1 month after operation,the Harris scores of two groups were higher than those before operation,and the Harris scores were higher with time,and those in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Compared with ALA with lateral femoris muscle incision,ALA without lateral femoris muscle incision is more conducive to reducing the patients'pain and facilitating the patients'faster recovery.
