摘 要:目的分析对于儿童急性呼吸道感染性疾病中,白细胞(White Blood Cell,WBC)、C反应蛋白(C-Reactive Protein,CRP)及淀粉样蛋白A(Serum Amyloid A,SAA)的鉴别诊断价值。方法选取2021年1月至2024年7月福建省漳州市第五医院收治的120例急性呼吸道感染儿童为研究对象,分为细菌感染组(n=64)和病毒感染组(n=56),择同期体检儿童为健康对照组(n=60)。比较WBC、CRP、SAA水平,并分析WBC、CRP、SAA单独及联合在急性呼吸道感染中的诊断效能。结果治疗前,细菌感染组研究对象WBC、CRP、SAA水平高于病毒感染组和健康对照组研究对象(P值均<0.05);病毒感染组研究对象WBC水平低于健康对照组(P<0.05),CRP和SAA水平高于健康对照组(P值均<0.05)。与治疗前相比,治疗后细菌感染组WBC、CRP、SAA水平明显降低(P<0.05),病毒感染组CRP、SAA水平明显降低(P<0.05),WBC水平明显升高(P<0.05)。WBC、CRP、SAA联合诊断的敏感度、特异度和准确性分别为89.17%、96.67%、91.67%。三者联合诊断儿童急性呼吸道感染的AUC值(0.918)最高;WBC诊断儿童急性呼吸道感染的AUC值为0.638;CRP诊断儿童急性呼吸道感染的AUC值为0.711;SAA诊断儿童急性呼吸道感染的AUC值为0.784。结论在儿童急性呼吸道感染的诊断中,WBC、CRP、SAA三者联合具有较高的诊断效能。Objective To analyze the diagnostic value of white blood cell(WBC)count,C-reactive protein(CRP),and serum amyloid A(SAA)in differentiating acute respiratory infections in children.Methods A total of 120 children diagnosed with acute respiratory infections from January 2021 to July 2024 at the Fifth Hospital of Zhangzhou,Fujian Province,were selected for this study.They were divided into a bacterial infection group(n=64)and a viral infection group(n=56),with a concurrent control group of healthy children(n=60).Levels of WBC,CRP,and SAA were compared and analyzed for their diagnostic performance both individually and in combination in acute respiratory infections.Results Before treatment,levels of WBC,CRP,and SAA in the bacterial infection group were significantly higher than those in the viral infection group and the healthy control group(all P<0.05).WBC levels in the viral infection group were lower than those in the healthy control group(P<0.05),while CRP and SAA levels were higher than those in the healthy group(both P<0.05).Compared to pre-treatment levels,post-treatment levels of WBC,CRP,and SAA significantly decreased in the bacterial infection group(P<0.05),and CRP and SAA levels significantly decreased while WBC levels significantly increased in the viral infection group(P<0.05).The sensitivity,specificity,and accuracy of the combined WBC,CRP,and SAA diagnosis were 89.17%,96.67%,and 91.67%,respectively.The combined diagnostic AUC value for acute respiratory infections in children was the highest at 0.918;the AUC values for WBC,CRP,and SAA alone were 0.638,0.711,and 0.784,respectively.Conclusion The combined use of WBC,CRP,and SAA provides high diagnostic efficacy in differentiating acute respiratory infections in children.
