电针八髎穴对糖尿病神经源性膀胱患者氧化应激、神经传导速度和尿流动力学的影响

摘  要:目的:观察电针八髎穴辅助治疗糖尿病神经源性膀胱对氧化应激、神经传导速度和尿流动力学的影响。方法:选取2021年4月—2023年6月在本院就诊的糖尿病神经源性膀胱患者70例随机分为治疗组和对照组,各35例。对照组予以常规治疗,治疗组在对照组基础上,予以电针八髎穴治疗。对比分析两组间氧化应激指标[治疗前后血清活性氧(ROS)、超氧化物歧化酶(SOD)及谷胱甘肽(GSH)]水平、感觉神经传导速度(SCV)、运动神经传导速度(MCV)、感觉神经传导潜伏时(LP of SC)、运动神经传导潜伏时(LP of MC)和尿流动力学(最大膀胱容量、膀胱排尿压力及残余尿量)等水平。结果:治疗后,两组血清ROS、SOD及GSH水平均较治疗前改善,差异具有统计学意义(P<0.05),且治疗组明显优于对照组,差异具有统计学意义(P<0.05);治疗后,两组SCV、MCV均较治疗前改善,差异具有统计学意义(P<0.05),且治疗组明显优于对照组(P<0.05);治疗后,两组LP of SC、LP of MC水平均较治疗前改善,差异具有统计学意义(P<0.05),且治疗组明显优于对照组,差异具有统计学意义(P<0.05);治疗后,两组患者最大膀胱容量、最大尿流速率和残余尿量均较治疗前改善,差异具有统计学意义(P<0.05),且治疗组明显优于对照组,差异具有统计学意义(P<0.05)。结论:电针八髎穴辅助治疗糖尿病神经源性膀胱可提高神经传导速度,改善尿流动力学,促进膀胱功能恢复,可能与抑制氧化应激反应有关。Objective:To observe the effects of electro-needling Baliao points on oxidative stress,nerve conduction velocity and urodynamics in treatment of diabetic neurogenic bladder.Methods:70 patients with diabetic neurogenic bladder were randomly divided into the treatment group and the control group,with 35 patients in each group.The control group received routine treatment;on which basis,the treatment group received electro-needling Baliao points.Oxidative stress indicators including serum reactive oxygen species(ROS),superoxide dismutase(SOD),glutathione(GSH),sensory nerve conduction velocity(SCV),motor nerve conduction velocity(MCV),sensory nerve conduction latency(LP of SC),motor nerve conduction latency(LP of MC),and urodynamics including maximum bladder volume,bladder urination pressure,and residual urine volume were compared between the two groups.Results:The serum levels of ROS,SOD and GSH were improved after the treatment compared to those before the treatment in the two groups(P<0.05),and the improvements were more significant in the treatment group than those in the control group(P<0.05).Both SCV and MCV were improved after the treatment compared to those before the treatment in the two groups(P<0.05),and the SCV and MCV were significantly better in the treatment group than those in the control group(P<0.05).The levels of LP of SC and LP of MC were improved after the treatment compared to those before the treatment in the two groups(P<0.05),and the improvements were more significant in the treatment group than those in the control group(P<0.05).The maximum bladder capacity,maximum urine flow rate,and residual urine volume were improved after the treatment compared to those before the treatment in the two groups(P<0.05),which were better in the treatment group than those in the control group(P<0.05).Conclusion:The auxiliary treatment of electro-needling Baliao points for diabetic neurogenic bladder can increase nerve conduction velocity,improve urodynamics,and promote the recovery of bladder function,which ma
