齐刺法深刺下关穴联合太乙神针治疗风寒型三叉神经痛临床研究

摘　　要：目的:对比齐刺法深刺下关穴联合太乙神针与传统针刺治疗风寒型三叉神经痛(TN)的临床疗效。方法:将60例风寒型TN患者随机分为观察组(30例)和对照组(30例)。观察组采用齐刺法深刺下关穴联合太乙神针治疗,对照组进行传统针刺治疗,所有患者针刺1次/d, 6次/周,共4周。对比两组患者治疗前后的简式麦吉尔疼痛问卷评分(VAS评分、PRI评分和PPI评分)、中医症候积分、临床总体印象评分(CGIS)和综合症状评分,比较治疗前后血清P物质(SP)、5-羟色胺(5-HT)及血清β-内啡肽(β-EP)的含量变化,观察并记录两组患者有无不良反应。结果:治疗后,观察组患者的VAS、 PPI、 PRI、中医症候积分及血清SP含量均较对照组低,差异有统计学意义(P<0.05);经治疗后,观察组患者血清5-羟色胺和β-内啡肽含量明显高于治疗前,差异有统计学意义(P<0.05),且高于对照组,差异有统计学意义(P<0.05)。两组患者均无严重不良反应发生。结论:齐刺法深刺下关穴配合太乙神针治疗风寒型三叉神经痛可减轻患者疼痛症状,明显优于常规针刺,其机制可能与血清P物质(SP)、血清β-内啡肽(β-EP)及5-羟色胺(5-HT)的含量变化有关。Objective:To compare the clinical effects of deep needling Xiaguan acupoint(ST7)combined with Taiyi miraculous moxa roll and conventional acupuncture in treatment of trigeminal neuralgia(TN)of wind-cold type.Methods:60 patients with TN of wind-cold type were randomly divided into the observation group(n=30)and the control group(n=30).The observation group was treated with deep triple needling ST7 combined with Taiyi miraculous moxa roll,and the control group was treated with conventional acupuncture.All the patients were treated once per day,six times per week for four weeks.The scores of simple McGill pain questionnaire(VAS score,PRI score,PPI score),TCM symptom score,clinical total impression score(CGIS),comprehensive symptom score,the serum levels of substance P(SP),5-hydroxytryptamine(5-HT)and beta-endorphine(β-EP)were compared between before and after the treatment in the two groups.The adverse reactions were recorded in both groups.Results:The scores of VAS,PPI,PRI and TCM symptoms,as well as the serum content of SP were lower in the observation group than those in the control group after the treatment(P<0.05).The serum contents of 5-HT andβ-EP were higher after the treatment than those before the treatment in the observation group(P<0.05),which were higher in the observation group than those in the control group after the treatment(P<0.05).There were no serious adverse reactions in both groups.Conclusion:The therapy of deep triple needling ST7 combined with Taiyi miraculous moxa roll can relieve the pain in the treatment of TN,which is superior to conventional acupuncture.The mechanism may be related to the changes of SP,β-EP and 5-HT.
