浅析《针灸大成》医案脾胃病诊疗特色

摘　　要：“脾胃为本”思想横贯中医学发展,脾胃系病证是中医内科学的重要组成部分。《针灸大成》卷九的31则医案中有11则与脾胃病相关。脾胃病医案中,杨氏运用五行生克诊疗疾病,依据卦象解释疾病本质,基于疾病病机病势辨证施治。诊治过程中杨氏力求分清标本,明辨缓急;调摄气机,秉轴持钧;穴少而巧,奇正相配;通调任督,选特定穴;针灸并用,注重手法。《针灸大成》医案脾胃病诊疗思路对现今针灸临床具借鉴价值和指导意义。The thought of"spleen and stomach is the foundation"runs through the development of traditional Chinese medicine.The disease and syndrome of spleen and stomach system is an important part of internal medicine of traditional Chinese medicine.Of the 31 medical cases of‘Acupuncture Dacheng'Volume 9,11 were related to spleen and stomach diseases.In the medical case of spleen and stomach disease,Yang used the fi ve elements to diagnose and treat the disease,explained the nature of the disease according to the hexagram,and treated the disease based on the pathogenesis of the disease.In the process of diagnosis and treatment,Yang distinguishes the specimens and priorities;regulates Qi;gets a small number of acupoints to choose but a clever idea,matches the extraordinary points and points of the right meridian;adjusts the conception vessel and governor vessel,selects specific acupoints;uses acupuncture and moxibustion together,pays attention to the technique.The dignosis and treatment ideas of spleen and stomach diseases in‘Acupuncture Dacheng'medical records have reference value and guiding signifi cance for today's acupuncture clinical practice.
