摘 要:目的探讨血清自噬相关蛋白Beclin1水平与糖尿病病人颈动脉粥样硬化和颈动脉内中膜厚度(CIMT)的相关性。方法前瞻性选取2017年3月至2020年12月于济南市妇幼保健院就诊的126例糖尿病病人为研究对象,就诊时测量病人血清Beclin1水平及CIMT水平,记录病人颈动脉粥样硬化情况,并根据病人血清Beclin1水平的中位数分为高水平组和低水平组;分析血清Beclin1水平与糖尿病颈动脉粥样硬化和CIMT水平的相关性;采用逐步回归评估CIMT的独立危险因子,构建受试者操作特征曲线以确定灵敏度和特异度。结果血清Beclin1水平的中位数为2.16μg/L,根据病人血清Beclin1水平的中位数分为高水平组和低水平组,每组63例。高水平组病人空腹血糖、糖化血红蛋白、三酰甘油、总胆固醇、低密度脂蛋白、CTMT[(5.89±0.95)mm比(7.46±1.10)mm]、颈动脉粥样硬化发生率[17.46%(11/63)比33.33%(21/63)]均低于低水平组,高密度脂蛋白高于低水平组(P<0.05);Pearson相关分析法分析显示血清Beclin1水平与糖尿病颈动脉粥样硬化和CIMT水平呈负相关(r=-0.42,P<0.001;r=-0.54,P<0.001),并与三酰甘油、总胆固醇、低密度脂蛋白也呈负相关(r=-0.42,P<0.001;r=-0.34,P<0.001;r=-0.52,P<0.001),与空腹血糖、糖化血红蛋白水平呈弱负相关(r=-0.31,P<0.001;r=-0.25,P=0.005);与高密度脂蛋白呈正相关(r=0.38,P<0.001);logistic回归分析显示血清Beclin1水平升高(OR=0.32)是CIMT>5 mm的独立保护因素,最佳截断值为2.35μg/L,曲线下面积为0.88,灵敏度和特异度分别为98.00%、70.00%。结论血清Beclin1水平与糖尿病病人颈动脉粥样硬化和CIMT水平呈负相关,CIMT>5 mm的最佳血清Beclin1截断值为2.35μg/L。Objective To investigate the correlation between serum autophagy-related protein Beclin1 level and carotid atherosclerosis and carotid intima-media thickness(CIMT)level in diabetic patients.Methods A total of 126 diabetic patients in Ji'nan Maternal and Child Health Hospital from March,2017 to December,2020 were prospectively selected as the research objects.The serum Beclin1 level and CIMT level of patients were measured at the time of treatment,and the carotid atherosclerosis of patients was recorded,and they were divided into high-level group and low-level group according to the median serum Beclin1 level of patients.The correlation between serum Beclin1 level and diabetic carotid atherosclerosis and CIMT level was analyzed;The independent risk factors of CIMT were evaluated by stepwise regression,and the receiver operation characteristic curve was constructed to determine the sensitivity and specificity.Results The median level of serum Beclin1 was 2.16μg/L,and patients were divided into high-level group and lowlevel group according to the median level of serum Beclin1,with 63 cases in each group.In the high level group,the fasting blood glucose,glycosylated hemoglobin,triglyceride,total cholesterol,low density lipoprotein,CIMT[(5.89±0.95)mm vs.(7.46±1.10)mm]and the incidence of carotid atherosclerosis[17.46%(11/63)vs.33.33%(21/63)]was lower than that of low-level group,the high-level HDL was higher than that in the low-level HDL group(P<0.05).Pearson correlation analysis showed that serum Beclin1 level was negatively correlated with diabetic carotid atherosclerosis and CIMT level(r=-0.42,P<0.001;r=-0.54,P<0.001),and was negatively correlated with triglyceride,total cholesterol and low density lipoprotein(r=-0.42,P<0.001;r=-0.34,P<0.001;r=-0.52,P<0.001),which was weakly negatively correlated with fasting blood glucose and glycosylated hemoglobin(r=-0.31,P<0.001;r=-0.25,P=0.005);It was positively correlated with high density lipoprotein(r=0.38,P<0.001).Logistic regression analysis showed that the increase o
