从“火”立论探讨《理虚元鉴》辨治肺癌癌因性疲乏的经验

摘　　要：通过分析《理虚元鉴》一书中对“虚劳六因”和“治虚三本二统论”理论的阐述,得出肺癌癌因性疲乏(cancer related fatigue,CRF)的病因病机可从“火”立论,总结为肺中伏火、火衰毒蕴、水火不济和血虚痰火四种。临证遵其“阴虚统于肺、阳虚统于脾”之旨,重视使用清金保肺、培土温中法,调治过程中当秉持“勿犯中州之土”“勿损至高之气”及“期年为限”的观点,为临床辨证论治肺癌CRF提供借鉴。By analyzing the statement on the theory of"six causes of consumptive disease"and"three-root and two-unity theory for the treatment of consumptive disease"in the book of Lixu Yuanjian,the causes and the pathogenesis of cancer related fatigue(CRF),based on"fire",could be divided into hidden fire in the lung,fire weakening and poison accumulation,disharmony between heart and kidney,blood deficiency and phlegm-fire.In clinical treatment,we should adhere to the rules of"Yin deficiency reflected in the lung,Yang deficiency reflected in the spleen",emphasizing the application of the methods of clearing lung to protect the lung,reinforcing the spleen and warming the middle energizer,we should hold the viewpoint of"don't damage the spleen and stomach""don't hurt the lung"and"years of treatment"in the process of regulation and treatment,which could provide the reference for syndrome differentiation and treatment of cancer related fatigue of lung cancer.
