健脾疏肝熄风汤治疗脾虚肝亢型小儿多发性抽动症临床研究

摘　　要：目的:探讨健脾疏肝熄风汤治疗脾虚肝亢型小儿多发性抽动症(multiple tourette disease,MTD)的临床疗效。方法:将62例MTD患儿随机分为对照组和观察组,每组各31例。对照组予硫必利片治疗,观察组在此基础上予健脾疏肝熄风汤内服治疗,两组均连续治疗12周。治疗结束后,比较两组耶鲁抽动症整体严重程度量表(yale global tic severity scale,YGTSS)评分、脾虚肝亢证单项症状评分的变化情况,以及临床疗效、中医证候疗效和不良反应发生情况。结果:治疗后两组YGTSS评分、脾虚肝亢证单项症状评分均降低(P<0.05),且观察组降低程度大于对照组(P<0.05);临床总有效率观察组[96.77%(30/31)]高于对照组[74.19%(23/31)](P<0.05);中医证候疗效总有效率观察组[100.00%(31/31)]高于对照组[80.65%(25/31)](P<0.05);不良反应发生率观察组[9.68%(3/31)]低于对照组[22.58%(7/31)](P<0.05)。结论:健脾疏肝熄风汤治疗脾虚肝亢型小儿MTD疗效明显,能控制疾病发展,改善中医证候,降低不良反应发生率。Objective:To survey curative effects of spleen-invigorating liver-soothing wind-extinguishing decoction in the treatment of pediatric multiple tourette disease(MTD)of spleen deficiency liver-hyperactivity pattern.Methods:All 62 MTD children were randomized into the control group and the observation group with 31 cases in each.The control group was treated with tiapride hydrochloride tablets,and the observation group took the decoction orally based on Western medicine,both groups were treated for 12 weeks continuously.By the end of the treatment,to compare the scores of YGTSS,the scores of single symptom of spleen deficiency liver-hyperactivity pattern,clinical effects,curative effects of TCM syndromes and the incidences of adverse reactions between both groups.Results:After the treatment,YGTSS scores and the scales of single symptom of spleen deficiency liver-hyperactivity pattern reduced in the two groups(P<0.05),and the decrease of the observation group was greater than that of the control group(P<0.05).Clinical total effective rate of the observation group was[96.77%(30/31)],higher than[74.19%(23/31)]of the control group(P<0.05);Total effective rate of the efficacy of TCM syndromes of the observation group was[100.00%(31/31)],higher than[80.65%(25/31)]of the control group;the incidences of adverse reaction of the observation group was[9.68%(3/31)],lower than[22.58%(7/31)]of the control group(P<0.05).Conclusion:Spleen-invigorating liver-soothing wind-extinguishing decoction could gain obvious clinical effects in the treatment of pediatric MTD of spleen deficiency liver-hyperactivity pattern,control the development of disease,improve TCM syndromes and lower the incidence of adverse reactions.
