摘 要:目的 研究不同宫颈病变患者高危型人乳头瘤病毒(HR-HPV)载量和调节性T淋巴细胞(Treg)表达差异。方法 选取2021年3月至2023年9月江苏省常熟市中医院200例HR-HPV持续感染患者,分为宫颈上皮内瘤变(CIN)组、宫颈癌组及宫颈炎组,检测并比较3组宫颈分泌物中HPV-DNA和Treg细胞比例。结果 宫颈炎组、CIN组Ⅰ级、CIN组Ⅱ级、CIN组Ⅲ级及宫颈癌组患者HR-HPV感染阳性率、HR-HPV载量及Treg细胞比例差异均有统计学意义(均P <0.05)。CIN组Ⅲ级、宫颈癌组患者中不同HR-HPV载量组间Treg细胞比例差异均有统计学意义(均P <0.05)。CIN组Ⅲ级及宫颈癌组患者HR-HPV载量与Treg细胞呈明显正相关(均P <0.05)。结论 HR-HPV载量与宫颈病变程度有关,宫颈微环境中的Treg细胞与HR-HPV载量共同参与宫颈病变的发生发展。Objective To investigate the differences in high-risk human papillomavirus(HR-HPV)load and regulatory T lymphocyte(Treg)expression among patients with different cervical lesions.Methods A total of 200 patients with HR-HPV persistent infection were divided into cervical intraepithelial neoplasia(CIN)group,cervical cancer group,and chronic cervicitis group.HPV-DNA in cervical secretions and Treg cells were detected and compared among the three groups.Results There were significant differences in the positive rate of HR-HPV infection,HR-HPV load,and Treg cells among the chronic cervicitis group,CIN gradeⅠgroup,CIN gradeⅡgroup,CIN gradeⅢgroup,and cervical cancer group(P<0.05).There was a significant difference in the proportion of Treg cells between different HR-HPV load groups in CIN gradeⅢand cervical cancer patients(all P<0.05).Treg cells were positively correlated with HR-HPV load in patients with CIN gradeⅢand cervical cancer(all P<0.05).Conclusion The viral load of HR-HPV is related to the degree of cervical lesions,and Treg cells in the cervical microenvironment and the viral load of HR-HPV jointly participate in the occurrence and development of cervical lesions.
