摘　　要：目的分析血清性激素、妊娠相关蛋白A(PAPP-A)、抗苗勒管激素(AMH)和β-人绒毛膜促性腺激素(β-HCG)对异位妊娠的预测价值。方法选取2018年1月至2022年1月江苏省如皋市人民医院接受治疗的89例异位妊娠患者为观察组,另选同期正常妊娠的63例健康女性为对照组。比较2组血清性激素[卵泡刺激素(FSH)、促黄体生成素(LH)、雌二醇(E2)、孕酮]、AMH、PAPP-A和β-HCG水平,并通过Logistic回归分析以上指标与异位妊娠的关系,采用受试者工作特征(ROC)曲线评估其对异位妊娠的诊断效能。结果观察组患者FSH和LH水平较对照组升高(P<0.05),E2、孕酮、AMH、PAPP-A和β-HCG水平低于对照组(P<0.05)。Logistic多因素分析显示,E2、孕酮、PAPP-A和β-HCG是异位妊娠的保护因素。ROC曲线分析结果显示,E2、孕酮、PAPP-A和β-HCG联合诊断异位妊娠的曲线下面积为0.976,诊断效能均高于4项单独诊断效能。结论血清E2、孕酮、PAPP-A和β-HCG联合对异位妊娠的发生有较高的诊断价值。Objective To analyze the predictive value of serumβ-human chorionic gonadotropin(β-HCG)in ectopic pregnancy patients.Methods The study involved 89 ectopic pregnancy patients treated from January 2018 to January 2022 as the observation group,and 63 healthy women with normal pregnancies during the same period as the control group.We collected general data for both groups and compared levels of serum sex hormones(FSH,LH,estradiol[E2],pro-gesterone),anti-Müllerian hormone(AMH),pregnancy-associated plasma protein-A(PAPP-A),andβ-HCG.Logistic re-gression analysis was used to explore the relationship between these markers and ectopic pregnancy,with diagnostic efficacy assessed using ROC curves.Results In the observation group,levels of FSH and LH were higher,while levels of E2,pro-gesterone,AMH,PAPP-A,andβ-HCG were lower than those in the control group,with statistically significant differences(P<0.05).Multifactor analysis showed that E2,progesterone,PAPP-A,andβ-HCG were protective factors for ectopic pregnancy.ROC curve analysis demonstrated that their combined area under the curve(AUC)was 0.976,showing superior diagnostic efficacy compared to individual markers.Conclusion Serum E2,progesterone,PAPP-A,andβ-HCG demonstrated significant predictive value for ectopic pregnancy,with their combined use providing higher diagnostic accuracy than individual markers.
