老年髋部骨折内固定术后废用性骨质疏松症发生现状及危险因素分析

摘　　要：目的探讨老年髋部骨折内固定术后废用性骨质疏松症(DOP)的发生现状和危险因素,为临床预防和治疗提供依据。方法将2022年1月至2023年10月延安大学附属医院收治的175例老年髋部骨折内固定术后患者纳入研究,根据术后随访6个月DOP发生情况分为DOP组与非DOP组,进行DOP发生现状和危险因素分析。结果175例患者中,内固定术后6个月DOP发生患者为54例,占30.9%。结果显示,年龄(OR=1.745,P<0.001)、性别(OR=2.649,P=0.016)、术后并发症(OR=2.724,P<0.001)是老年髋部骨折内固定术患者术后发生DOP的独立因素,而术后早期抗阻力训练为保护因子(OR=0.416,P=0.019)。结论年龄大、女性、合并术后并发症是内固定术后DOP发生的危险因素,而术后进行早期抗阻力训练是其保护因素。Objective To investigate the incidence and risk factors of disuse osteoporosis(DOP)after internal fixation for elderly hip fractures,and to provide evidence for clinical prevention and treatment.Methods A total of 175 elderly patients with hip fractures who underwent internal fixation surgery at the Affiliated Hospital of Yan'an University from January 2022 to October 2023 were included in the study.They were divided into the DOP group and the non-DOP group according to the occurrence of DOP at a half-year follow-up.The DOP occurrence status and risk factors were analyzed.Results Among the 175 patients,54 cases(30.9%)had DOP in the first half-year after internal fixation.The results showed that age(OR=1.745,P<0.001),gender(OR=2.649,P=0.016),and postoperative complications(OR=2.724,P<0.001)were independent factors for postoperative DOP in elderly patients with internal fixation of hip fractures.Early resistance training was a protective factor(OR=0.416,P=0.019).Conclusions Advanced age,female gender,and postoperative complications were the risk factors for DOP after internal fixation,while early resistance training was a protective factor.
