彩色超声多普勒引导下右颈内静脉穿刺术对手术麻醉患者穿刺准确性及并发症的影响

摘　　要：目的综合分析对中心静脉置管手术麻醉患者实施彩色超声多普勒引导下右颈内静脉穿刺的效果,及对穿刺准确性、并发症的影响。方法研究选择泉州东南医院2020年1月至2023年8月需行中心静脉置管手术的患者86例为对象,按照随机数表法将其分成对照组与观察组,各43例,在麻醉中对照组采用常规盲探法右颈内静脉穿刺术,观察组采用彩色超声多普勒引导下右颈内静脉穿刺术,对比两组穿刺时间、一针穿刺成功与并发症情况、操作者满意度。结果观察组的一针穿刺成功率(93.02%vs.74.42%)与操作者满意度(97.67%vs.83.72%)比对照组更高(P<0.05);观察组的穿刺时间短于对照组(P<0.05);在穿刺并发症总发生率方面,观察组比对照组(4.65%vs.18.60%)更低(P<0.05)。结论中心静脉置管患者在彩色多普勒超声引导下实施右颈内静脉穿刺,具有较高的穿刺成功效果,且有利于缩短穿刺时间,并可预防并发症,最终促进手术的顺利进行。Objective To comprehensively analyze the effect of color Doppler guided right internal jugular vein puncture on anesthesia patients undergoing central venous catheterization surgery,and its impact on puncture accuracy and complications.Methods A total of 86 patients who required central venous catheterization surgery at Quanzhou Southeast Hospital from January 2020 to August 2023 were selected as the study subjects.They were randomly divided into a control group and an observation group,with 43 cases in each group.During anesthesia,the control group underwent conventional blind probing right internal jugular vein puncture,while the observation group underwent color Doppler guided right internal jugular vein puncture.The puncture time,success and complications of one needle puncture,and operator satisfaction were compared between the two groups.Results The success rate of needle puncture in the observation group(93.02%vs.74.42%)and operator satisfaction(97.67%vs.83.72%)were higher than those in the control group,and the difference was statistically significant(P<0.05),The puncture time of the observation group was lower than that of the control group(P<0.05),In terms of overall incidence of puncture complications,the observation group had a lower incidence rate than the control group(4.65%vs.16.80%)(P<0.05).Conclusions Under the guidance of color Doppler ultrasound,patients with central venous catheterization can undergo right internal jugular vein puncture,which has a high success rate and is beneficial for shortening the puncture time,preventing complications,and ultimately promoting the smooth progress of the surgery.
