摘 要:目的:基于动脉自旋标记成像技术(ASL)探讨坏死性小肠结肠炎(NEC)对早产儿脑血流量的影响。方法:选取2022年1月至2023年6月在郑州大学第三附属医院新生儿重症监护室住院治疗的37例NEC早产儿,按胎龄(±3 d)、出生体重(±300 g)1∶1匹配同期未患NEC的早产儿作为对照;所有早产儿均于纠正胎龄35~40周完善头颅常规MRI及ASL检查,获取脑血流值。应用线性回归分析NEC对不同脑功能区脑血流值的影响。2组患儿均于矫正胎龄40~44周时进行NBNA评分。结果:控制混杂因素后,NEC可升高早产儿右侧额叶、双侧颞叶、右侧枕叶、双侧基底神经节及双侧丘脑的脑血流值[β′(95%CI)分别为0.281(0.267~6.117)、0.234(0.055~4.088)(左侧)、0.318(0.601~5.736)(右侧)、0.272(0.234~8.523)、0.386(1.702~8.032)(左侧)、0.377(1.690~8.755)(右侧)、0.389(2.297~10.890)(左侧)、0.318(0.919~8.884)(右侧),P<0.05]。纠正胎龄40~44周,NEC组和对照组NBNA评分分别为36(34,37)、37(35,38)分,差异无统计学意义(P=0.187)。结论:早产儿发生NEC后脑血流值增高。Aim:To investigate the effects of necrotizing enterocolitis(NEC)on cerebral blood flow volume in preterm infants based on arterial spin labeling imaging(ASL).Methods:Thirty-seven preterm infants with NEC who were hospitalized in the Neonatal Intensive Care Unit of the Third Affiliated Hospital of Zhengzhou University from January 2022 to June 2023 were selected.Preterm infants with NEC and those without were matched in a 1∶1 ratio regarding gestational age(±3 days)and birth weight(±300 g).All preterm infants underwent cranial routine MRI and ASL examination at 35-40 weeks of corrected gestational age,and cerebral blood flow value was obtained.The effects of NEC on cerebral blood flow value in different brain functional areas were analyzed by linear regression analysis.NBNA score at the corrected gestational age of 40-44 weeks was assessed.Results:After adjusting for confounding factors,NEC could increase the cerebral blood flow value in the preterm infants at right frontal lobe,bilateral temporal lobe,right occipital lobe,bilateral basal ganglion and bilateral thalamus[β′(95%CI)was 0.281(0.267-6.117),0.234(0.055-4.088),0.318(0.601-5.736),0.272(0.234-8.523),0.386(1.702-8.032),0.377(1.690-8.755),0.389(2.297-10.890),0.318(0.919-8.884),P<0.05].There was no significant difference in NBNA score at the corrected gestational age of 40-44 weeks between NEC group and control group[36(34,37),37(35,38),P=0.187].Conclusion:Cerebral blood flow volume increases in preterm infants with NEC.
