摘 要:目的研究鼻负压置换联合机械辅助排痰氧气雾化治疗小儿肺炎的临床疗效。方法选取2022年3月-2023年9月在我院诊治的72例小儿肺炎患儿为研究对象,采用随机数字表法分为对照组和研究组,各组36例。对照组采用机械辅助排痰氧气雾化治疗,研究组在对照组基础上联合鼻负压置换治疗。比较两组治疗效果、临床症状(咳嗽、体温、肺部啰音)消失时间、血气指标[氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、pH]、炎性因子[C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素-6(IL-6)]水平。结果研究组治疗总有效率(94.44%)高于对照组(83.33%)(P<0.05);研究组咳嗽消失、体温恢复正常、肺部啰音消失时间均短于对照组(P<0.05);两组治疗后PaO_(2)、pH高于治疗前,PaCO_(2)低于治疗前,且研究组PaO_(2)、pH高于对照组,PaCO_(2)低于对照组(P<0.05);两组治疗后CRP、PCT、IL-6水平均低于治疗前,且研究组低于对照组(P<0.05)。结论鼻负压置换联合机械辅助排痰氧气雾化治疗小儿肺炎临床效果理想,可提高总有效率,改善血气指标,减轻炎症反应,利于患儿临床症状的快速消失,值得临床加以应用。Objective To study the clinical efficacy of nasal negative pressure replacement combined with mechanical assisted sputum oxygen atomization in the treatment of children with pneumonia.Methods A total of 72 children with pneumonia diagnosed and treated in our hospital from March 2022 to September 2023 were selected as the research objects.They were divided into control group and study group by random number table method,with 36 children in each group.The control group was treated with mechanical assisted expectoration oxygen atomization,and the study group was treated with nasal negative pressure replacement on the basis of the control group.The treatment effect,disappearance time of clinical symptoms(cough,body temperature,lung rale),blood gas indexes[oxygen partial pressure(PaO_(2)),carbon dioxide partial pressure(PaCO_(2)),pH],inflammatory factors[C-reactive protein(CRP),procalcitonin(PCT),interleukin-6(IL-6)]were compared between the two groups.Results The total effective rate of treatment in the study group(94.44%)was higher than that in the control group(83.33%)(P<0.05).The time of cough disappearance,body temperature recovery and lung rale disappearance in the study group were shorter than those in the control group(P<0.05).After treatment,PaO_(2) and pH in the two groups were higher than those before treatment,PaCO_(2) was lower than that before treatment,and PaO_(2) and pH in the study group were higher than those in the control group,PaCO_(2) was lower than that in the control group(P<0.05).After treatment,the levels of CRP,PCT and IL-6 in the two groups were lower than those before treatment,and those in the study group were lower than those in the control group(P<0.05).Conclusion The clinical effect of nasal negative pressure replacement combined with mechanical assisted expectoration and oxygen atomization in the treatment of pediatric pneumonia is ideal,which can improve the total effective rate,improve the blood gas index,reduce the inflammatory reaction,and is conducive to the rapid disappearance o
