腹腔镜下左右手打结缝合在子宫全切术中的应用价值探究

摘　　要：目的:探究腹腔镜下左右手打结缝合在子宫全切术中的应用价值。方法:选取2021年4月—2023年11月于玉林市第一人民医院拟行腹腔镜下子宫全切术的患者90例为研究对象,随机分为传统组、双极电凝组、左右手组,各30例。传统组实施腔内传统打结缝合,双极电凝组实施双极凝闭血管,左右手组实施左右手打结缝合。对比三组手术情况、疼痛评分、生活质量。结果:左右手组手术时间、切口愈合时间、住院时间短于传统组与双极电凝组,术中出血量少于传统组与双极电凝组(P<0.05);术后1、3 d,左右手组视觉模拟评分法评分低于传统组与双极电凝组(P<0.05);术后,三组各项世界卫生组织生活质量测定简表评分升高,左右手组生活质量评分均高于传统组、双极电凝组(P<0.05)。结论:在腹腔镜下行子宫全切术中,对出血点进行左右手打结缝合子宫血管后再切断,可显著减少术中出血量,缩短手术时间,减轻患者疼痛,提高生活质量。Objective:To explore the application value of laparoscopic knotting and suture in total hysterectomy.Methods:A total of ninety patients who planned to undergo total laparoscopic hysterectomy in Yulin First People's Hospital from April 2021 to November 2023 were selected as the study objects and randomly divided into traditional group,bipolar electrocoagulation group and left and right hand group,with thirty cases in each group.Traditional intracavitary knotting suture was performed in the traditional group,bipolar coagulation was performed in the bipolar group,and left-handed and left-handed knotting suture was performed in the left and right hand group.The operation,pain score and quality of life of the three groups were compared.Results:The operation time,incision healing time and hospital stay of the left and right hand groups were shorter than those of the traditional and bipolar electrocoagulation groups,and the intraoperative blood loss was less than that of the traditional and bipolar electrocoagulation groups(P<0.05);on the 1st and 3rd day after operation,the visual analog scores of the left and right hand groups were lower than those of the traditional group and bipolar electrocoagulation group(P<0.05);after operation,the scores of the world health organization QOL in the three groups were increased,and the scores of the left and right hand groups were higher than those in the traditional group and the bipolar electrocoagulation group(P<0.05).Conclusion:In laparoscopic total hysterectomy,the bleeding points were knotted and sutured with the right and left hands of the uterine vessels,and then cut off,which can significantly reduce the amount of intraoperative bleeding,shorten the operation time,reduce the pain of patients,and improve the quality of life.
