无创呼吸机早期介入对重症肺炎患儿血气分析指标改善情况的动态观察

摘  要:目的:评估无创呼吸机(NIV)早期介入对重症肺炎患儿血气分析指标的改善效果。方法:选取2023年1月—2024年1月仁怀市人民医院收治的重症肺炎患儿206例进行研究,随机分为两组,各103例。对照组采用常规治疗,NIV组在对照组基础上采用NIV早期介入治疗。比较两组治疗前后血气分析及相关指标变化情况、住院时间、并发症发生情况。结果:治疗2、4、8、24、48、72 h后,两组动脉血氧分压(PaO_(2))、pH值升高,动脉血二氧化碳分压(PaCO_(2))降低,NIV组PaO_(2)、PaCO_(2)优于对照组(P<0.05);治疗24、48、72 h后,NIV组pH值优于对照组(P<0.05)。治疗2、4、8、24、48、72 h后,两组呼吸频率、心率下降,氧合指数升高,NIV组呼吸频率、心率优于对照组(P<0.05);治疗24、48、72 h后,NIV组氧合指数优于对照组(P<0.05)。NIV组住院时间短于对照组(P<0.001)。两组并发症总发生率比较,无统计学差异(P>0.05)。结论:无创呼吸机早期介入可改善重症肺炎患儿血气分析指标,缓解其呼吸窘迫,提高氧合,缩短住院时间,且安全性较高。Objective:To evaluate the improvement effect of early intervention of non-invasive ventilator(NIV)on blood gas analy⁃sis indexes in children with severe pneumonia.Methods:A total of two hundred and six children with severe pneumonia admitted to Renhuai People's Hospital from January 2023 to January 2024 were randomly divided into two groups,one hundred and three cases each.The control group received conventional treatment,and the NIV group received early interventional treatment on the basis of the control group.The changes of blood gas analysis and related indexes,hospital stay and complications were compared between the two groups before and after treatment.Results:After treatment for 2,4,8,24,48 and 72 h,the partial pressure of oxy⁃gen(PaO_(2))and pH value were increased,and the partial pressure of carbon dioxide(PaCO_(2))was decreased in both groups,and the PaO_(2)and PaCO_(2)in NIV group were better than those in control group(P<0.05);after 24,48 and 72 h of treatment,pH value in NIV group was better than that in control group(P<0.05).After 2,4,8,24,48 and 72 h treatment,the respiratory rate and heart rate of the two groups decreased,and the oxygenation index increased,and the respiratory rate and heart rate of the NIV group were better than those of the control group(P<0.05).After 24,48 and 72 h treatment,the oxygenation index of NIV group was better than that of control group(P<0.05).The length of hospital stay in NIV group was shorter than that in control group(P<0.001).There was no sig⁃nificant difference in the total incidence of complications between the two groups(P>0.05).Conclusion:Early intervention of nonin⁃vasive ventilator can improve blood gas analysis index,relieve respiratory distress,improve oxygenation,shorten hospital stay,and have high safety.
