摘　　要：目的:分析上海市公立医疗机构互联网门诊医疗服务提供现状,探索影响因素,并提出相关建议。方法:通过回顾性研究,调研上海市15所三级公立医院2021-2023年互联网门诊医疗服务人次、提供诊疗服务的医师人数、互联网诊疗职能设置、线上结算功能以及科普宣教情况,形成面板数据。以互联网门诊服务人次为因变量,医师人数、互联网诊疗职能设置情况、线上结算功能以及科普宣教作为自变量,利用固定效应模型进行回归分析。结果:2022年互联网门诊服务人次最多,2023年较2021年有所上升。医院互联网职能设置及线上结算功能逐年优化,影响互联网门诊服务的主要因素为医师人数(P<0.05)和是否支持线上异地结算功能(P<0.05)。结论:2021-2023年上海市公立三级医疗机构互联网门诊整体服务情况较好,医疗机构根据医务人员排班合理安排工作时间线上就诊,优化信息系统设置,进一步完善支持互联网异地患者直接就诊。此外,充分发挥医院互联网诊疗管理机构的职能定位,结合新媒体形式下的医学科普宣教提升医院综合影响力,吸引更多的患者利用互联网诊疗模式就医。Objective:To analyze the current status of internet outpatient services provided by public medical institutions in Shanghai,explore influencing factors,and provide relevant recommendations.Methods:A retrospective study was conducted,surveying 15 Grade III public hospitals in Shanghai from 2021 to 2023.Data was collected on the number of internet outpatient visits,the number of physicians providing services,the establishment of internet healthcare functions,online settlement features,and health education efforts,forming a panel dataset.The number of internet outpatient visits was used as the dependent variable,while physician numbers,internet function setup,online settlement capabilities,and health education were used as independent variables.A fixed-effects model was used for regression analysis.Results:The highest number of internet outpatient visits occurred in 2022,with an increase in 2023 compared to 2021.The hospitals'internet functions and online settlement capabilities improved year by year.The main factors influencing internet outpatient services were the number of physicians(P<0.05)and the availability of online cross-regional settlement features(P<0.05).Conclusion:From 2021 to 2023,the overall performance of internet outpatient services in Shanghai's Grade III public hospitals was good.Hospitals should schedule online consultations based on the availability of medical staff,optimize information systems,and further improve support for crossregional internet consultations.Additionally,hospitals should enhance the role of internet healthcare management departments and leverage new media for medical education to increase the hospital's comprehensive influence and attract more patients to use internet healthcare services.
