摘　　要：中医药辨证治疗难治性慢性咳嗽临床疗效显著,且安全性高。临床常用药物包括钩藤、蝉蜕等中药单体,小柴胡汤、麻黄细辛附子汤等古方,彭氏四味方、伤寒温病合方等自拟方,小儿消积止咳口服液等中成药。但目前中医药治疗难治性慢性咳嗽的研究仍存在一些不足,如临床辨证标准尚未统一,缺乏客观、规范的中医证候诊断标准;临床研究不完善,缺乏高级别循证医学证据支持的治疗方案;作用机制不明确,缺乏病证结合模式下中医药作用机制的研究等,亟需开展难治性慢性咳嗽证候诊断标准的研究,加强全国范围的多中心、大样本临床调查,同时借鉴现代科学方法,促进动物模型中医证型的建立,创新实验研究方法,深入挖掘中医药作用机制,为该病的防治提供新思路。The clinical efficacy and safety of traditional Chinese medicine in treatment of refractory chronic cough are remarkable.The TCM monomers of Gouteng(Uncaria)and Chantui(Cicada slough),the classic formulas such as Xiao Chaihu Decoction and Mahuang Fuzi Xixin Decoction,the self-proposed formulas such as Peng′s Siwei Formula and Shanghan Wenbing combination Formula,and the proprietary medicines such as Xiaoer Xiaoji Zhike Oral Liquid are quite effective in clinical practice.At present,the following problems still exist in the research of TCM treatment of refractory chronic cough:the clinical identification criteria have not been unified,and there is a lack of objective and standardized diagnostic criteria for TCM symptoms;clinical research is not perfect,and there is a lack of treatment plans supported by high-level evidence-based evidence;the mechanism of action is not clear,and there is a lack of research on the therapeutic mechanism of TCM in the mode of combining illnesses with evidence,etc.,so it is urgent to carry out the research on diagnostic criteria for symptoms of refractory chronic cough and strengthen the research on the diagnostic criteria of TCM.There is an urgent need to carry out research on the diagnostic criteria of refractory chronic cough,strengthen the nationwide multi-center and large-sample clinical investigation,and at the same time,draw on modern scientific methods to promote the establishment of TCM animal models with evidence,innovate the experimental research methods,and deeply excavate the mechanism of action of TCM,so as to provide a new idea for the prevention and treatment of this disease.
