摘　　要：国医大师朱良春教授认为免疫性血小板减少性紫癜核心病因病机为:早期多属血热胃火,中期恒见阴虚内热,后期则多为脾肾阳虚,初病多实,久病多虚。针对病因病机创制“炙牛角[角思]汤”。牛角[角思]烧炭后性涩,善收敛止血、止带、止遗、止痢,敛正气而不敛邪;生用,其苦温之性,化瘀血而不伤新血,出血诸症有瘀血者多用之;其炙后,温补之性,可养肝肾之血,填肾督之精,补冲任之虚,修经络之损。临床可随症加减:肾阳偏虚者加淫羊藿;脾阳虚者加炮姜炭、炒白术;气虚明显者加黄芪、党参;血虚明显者加当归、阿胶。Professor Zhu Liangchun,the National TCM Master,believes that the core etiology and pathogenesis of immune thrombocytopenic purpura is as follows:blood-Heat and Stomach Fire in the early stage,Yin deficiency and internal Heat in the middle stage and Spleen and Kidney Yang deficiency in the late stage.The disease is mostly with excess syndrome at the beginning and deficiency syndrome in the long term.Therefore,for the etiology and mechanism of the disease,Zhi Niujiaosai Decoction is created.The charking of the bone in ox horn is astringent in character and is good at stopping bleeding,stopping the leucorrhea,stopping the enuresis and stopping catharsis,with convergence of Zheng Qi but not convergence of the evil factors;the raw product of it can remove blood stasis without hurting the new blood,with bitter and warm in nature,so it is often used to treat bleeding conditions with stasis of blood;the roasted production of it is warm in nature and can tonify the blood in Liver and Kidney,increase the Essence in Du meridian,and can tonify the deficiency of Chong meridian and Ren meridian by reducing the injury in meridians.Clinically,the prescription can be modified according to the symptoms:if the Kidney Yang is weak,add Yinyanghuo(Epimedium);if the Spleen yang is weak,add Paojiangtan(Ginger charcoal)and Chaobaizhu(Fried atractylodes);for severe Qi deficiency,add Huangqi(Astragalus),Dangshen(Codonopsis pilosula);for obvious blood deficiency,add Danggui(Radix angelicae sinensis)and Ejiao(Colla corii asini).
