通脉化斑汤对冠心病患者症状、血管内皮功能及心肺功能的影响

摘　　要：目的:观察通脉化斑汤治疗冠状动脉粥样硬化性心脏病(简称冠心病)的临床疗效,并观察其对患者症状、血管内皮功能、心肺功能的影响。方法:80例冠心病患者随机分为对照组和研究组。对照组给予传统疗法,研究组在此基础上给予通脉化斑汤,两组均连续治疗6个月。比较两组患者临床疗效、中医证候积分、血管内皮指标[血浆内皮素-1(endothelin-1,ET-1)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、血清一氧化氮(nitric oxide,NO)水平]和心肺功能[用力肺活量(forced vital capacity,FVC)和左室射血分数(left ventricular ejection fraction,LVEF)]。结果:研究组有效率为92.5%,对照组有效率为75.0%,两组有效率比较,差异有统计学意义(P<0.05)。治疗后,两组患者胸痛或胸闷、神疲乏力、气短或气促、心悸评分均显著降低(P<0.05),研究组以上指标均低于同期对照组(P<0.05)。治疗后,两组患者VEGF、NO水平显著升高(P<0.05),ET-1水平显著降低(P<0.05);研究组VEGF、NO水平高于同期对照组(P<0.05),ET-1水平低于同期对照组(P<0.05)。治疗后,两组患者FVC、LVEF显著升高(P<0.05),研究组以上指标高于同期对照组(P<0.05)。结论:通脉化斑汤治疗冠心病临床疗效显著,可显著改善患者临床症状、血管内皮功能和心肺功能。Objective:To observe the clinical efficacy of Tongmai Huabang Decoction in treatment of coronary atherosclerotic heart disease(coronary heart disease)and its effects on symptoms,endothelial function and cardiopulmonary function of patients.Methods:80 patients with coronary heart disease were randomly divided into the control group and the study group.The control group was treated with traditional therapy,and the study group was treated with Tongmai Huabang Decoction in addition to that of the control group.Both groups were treated continuously for 6 months.The two groups were compared in terms of clinical efficacy,TCM points,vascular endothelial indexes[plasma endothelin-1(ET-1),vascular endothelial growth factor(VEGF),serum nitric oxide(NO)]and cardiopulmonary function[forced vital capacity(FVC)and left ventricular ejection fraction(LVEF)].Results:The effective rate was 92.5%in the study group and 75.0%in the control group.The difference was statistically significant in the efficiency of the two groups(P<0.05).After treatment,chest pain or chest tightness,fatigue,shortness of breath or shortness of breath,and palpitation scores were significantly reduced in both groups(P<0.05),and the above indexes in the study group were lower than those in the control group in the same period(P<0.05).After treatment,VEGF and NO levels were significantly higher(P<0.05)and ET-1 levels were significantly lower(P<0.05)in both groups;VEGF and NO levels in the study group were higher than those in the control group during the same period(P<0.05),and ET-1 levels were lower than those in the control group during the same period(P<0.05).After treatment,FVC and LVEF were significantly higher in both groups(P<0.05),and the above indexes in the study group were higher than those in the control group during the same period(P<0.05).Conclusion:The clinical efficacy of Tongmai Huaban Decoction in treatment of coronary artery disease is remarkable,which can significantly improve the clinical symptoms,vascular endothelial function and cardiopul
