超声引导下胸椎旁神经阻滞复合静脉麻醉在胸腔镜肺癌根治术中的应用研究

摘　　要：探讨超声引导下胸椎旁神经阻滞复合静脉麻醉在胸腔镜肺癌根治术中的应用效果。选取河北北方学院附属第一医院收治的70例行胸腔镜肺癌根治术的患者为研究对象,采用随机数字表法将其分为观察组和对照组两组(n=35)。观察组实施超声引导下胸椎旁神经阻滞复合静脉麻醉,对照组实施全凭静脉麻醉,对比两组患者各指标变化。结果表明,观察组T_(1)、T_(2)、T_(3)时刻HR和MAP的变化幅度均显著低于对照组(P<0.05);观察组术后不同时刻的VAS评分均低于对照组(P<0.05);观察组围手术期麻醉药物使用量低于对照组(P<0.05)。研究发现,超声引导下胸椎旁神经阻滞复合静脉麻醉的术后镇痛效果显著,同时可减少患者围手术期麻醉药物用量并降低术后不良事件发生率,有利于患者早期恢复。To investigate the application effect of ultrasound-guided thoracic paravertebral nerve block combined with intravenous anesthesia in thoracoscopic radical lung cancer surgery.A total of 70 patients who underwent thoracoscopic radical lung cancer surgery in the First Affiliated Hospital of Hebei North University were selected as the study subjects,and they were divided into observation group and control group by random number table method(n=35).Ultrasound-guided thoracic paravertebral nerve block combined intravenous anesthesia was used in the observation group,and intravenous anesthesia was used in the control group,and the changes of various indexes between the two groups were compared.The results showed that the changes of HR and MAP at T_(1)、T_(2) and T_(3) in the observation group were significantly lower than those in the control group(P<0.05),the VAS score in the observation group at different times after surgery was lower than that in the control group(P<0.05),the use of perioperative anesthetic drugs in the observation group was lower than that in the control group(P<0.05).It was found that ultrasound-guided thoracic paravertebral nerve block combined with intravenous anesthesia had a significant postoperative analgesic effect,and at the same time,it could reduce the amount of perioperative anesthetic drugs and the incidence of postoperative adverse events,which was conducive to early recovery.
