摘 要:目的了解河南省2023年急性弛缓性麻痹(Acute Flaccid Paralysis,AFP)病例中脊髓灰质炎(脊灰)病毒VP1区核苷酸突变个数、突变位点及其氨基酸变异情况。方法对AFP病例中分离到的11株脊灰病毒进行VP1区核苷酸序列测定和分析,使用Contig Express软件对序列进行拼接,使用MEGA软件对序列进行比对分析。结果通过Realtime RT-PCR鉴定的11株脊灰病毒,均为疫苗相似株(Sabin-Like,SL),1株为SLIII,3株为SLI,7株为SLI+SLIII,其中2023-506F1 III型为6个核苷酸变异,是疫苗高变异株脊灰病毒(Vaccine-hypervariable Poliovirus,VHPV)。10株SLI毒株与SLI参考株相比,发现nt2749是常见突变位点,同时导致编码氨基酸的改变;与WPVI参考株相比,nt2749为毒力回复突变。8株SLIII毒株与SLIII参考株相比,发现nt2832、nt3353和nt3357为常见突变位点,同时导致编码氨基酸的改变;与WPVIII参考株相比,nt2832和nt3301为毒力回复突变。结论河南省2023年AFP中分离到的11株脊灰病毒均为疫苗相似株,某些位点发生毒力回复突变。Objective To analyze the number of mutations,mutation sites,and amino acid variations in the VP1 region of poliovirus in acute flaccid paralysis(AFP)cases in Henan Province in 2023.Methods VP1 nucleotide sequencing and analysis were performed on 11 strains of poliovirus isolated from AFP using Contig Express software;Multi sequence alignment using MEGAsoftware.Results The 11 isolated poliovirus strains were identified by Real time RT PCR,all of which were vaccine like strains(Sabin Like,SL),with 1 strain being SLIII,3 strains being SLI,and 7 strains being SLI+SLIII.Among them,2023-506F1 type III had 6 nucleotide mutations,making it a vaccine highly mutated strain of poliovirus(VHPV).Among 10 strains of SLI,compared with the SLI reference strain,nt2749 was found to be a common mutation site,leading to changes in the coding amino acids;compared with WPVI reference strain,nt2749 was found to be a toxicity recovery mutation.Among 8 strains of SLIII,compared with the SLIII reference strain,nt2832,nt3353,and nt3357 were found to be common mutation sites,leading to changes in the coding amino acids;compared with WPVIII reference strain,nt2832 and nt3301 were found to be toxicity recovery mutations.Conclusions The 11 strains of poliovirus isolated from AFP in Henan Province in 2023 are all vaccine like strains.Virulence recovery mutations occur at certain sites.
