不同血清学检测方法对人布鲁氏菌病诊断效能的比较研究

摘  要:目的对比虎红平板凝集试验(RBT)、试管凝集试验(SAT)和间接酶联免疫吸附试验(IELISA)在人布鲁氏菌病诊断中的应用,为科学诊断提供技术依据。方法收集2021年1—12月平顶山市疾病预防控制中心布鲁氏菌病门诊就诊的疑似布病患者血清,对血清标本运用RBT、SAT和IELISA方法同时检测,以《布鲁氏菌病诊疗指南》中布鲁氏菌病确诊方法为诊断金标准将血清学检测结果与金标准进行比较,评价各组检测方法对人布鲁氏菌病的诊断价值。结果共采集228份血清标本,RBT、SAT及IELISA血清学检测阳性率分别为42.98%(98/228)、33.77%(77/228)和45.18%(103/228),RBT与IELISA的阳性率差异无统计学意义(χ^(2)=0.222,P>0.05),SAT的阳性率均低于RBT(χ^(2)=6.921,P<0.05)和IELISA(χ^(2)=6.205,P<0.05);IELISA试验的灵敏度、特异度、准确率、约登指数和ROC曲线下面积最大,分别为89.90%、89.15%、89.47%、0.503和0.751。结论RBT、SAT和IELISA血清学检测方法均对人布鲁氏菌病检测有一定的价值,根据不同情况,建议RBT和IELISA的联合应用,可简便、快速、准确的检测布鲁氏菌抗体,为布鲁氏菌病的防治提供重要技术支撑,值得推广应用。Objective To compare the application of Rose Bengal plate agglutination test(RBT),tube agglutination test(SAT)and indirect enzyme linked immunosorbent assay(IELISA)in the diagnosis of human brucellosis,and provide technical basis for scientific diagnosis.Methods The serum samples from suspected brucellosis patients who visited the brucellosis clinic of Pingdingshan Center for Disease Control and Prevention were collected from January to December 2021,and the serum samples were tested by RBT,SAT and IELISA methods at the same time.The diagnostic method of brucellosis in the Guidelines for the Diagnosis and Treatment of Brucellosis was used as the diagnostic gold standard.Serological test results were compared with the gold standard,and the diagnostic value of each test method for human brucellosis was evaluated.Results A total of 228 serum samples were collected.The results of RBT,SAT,and IELISA serological tests were 42.98%(98/228),33.77%(77/228)and 45.18%(103/228),respectively.There was no significant difference in the positive rates between RBT and IELISA(χ^(2)=0.222,P>0.05),and the positive rate for SAT was lower than those for RBT(χ^(2)=6.921,P<0.05)and IELISA(χ^(2)=6.205,P<0.05).The sensitivity,specificity,accuracy,Youden index and area under ROC curve for IELISA test were largest,which were 89.90%,89.15%,89.47%,0.503 and 0.751,respectively.Conclusions The three serological detection methods of RBT,SAT and IELISA are all of certain value in the detection of human brucellosis.According to different situations,it is suggested to take the combined application of RBT and IELISA,which can detect brucellosis antibody simply,quickly and accurately,provide important technical support for the prevention and treatment of brucellosis,and worth popularizing and applying.
