摘　　要：目的探讨家庭医生签约服务模式对高血压伴微量白蛋白尿(MAU)患者血压控制、尿蛋白水平、生活质量及用药依从性的影响。方法120例高血压伴MAU患者随机分为管理组和对照组各60例,分别给予家庭医生签约服务和常规社区管理。比较2组患者干预前后血压水平、尿蛋白水平、生活质量及用药依从性的变化。结果干预后,2组患者的血压水平均较干预前有所下降(P<0.05);与对照组相比,管理组的血压控制效果更好,收缩压和舒张压均显著低于对照组(P<0.001);管理组患者的尿蛋白/肌酐比值低于对照组,生活质量评价高于对照组,差异有统计学意义(均P<0.001)。结论家庭医生签约服务模式能够有效改善高血压伴MAU患者的血压、尿蛋白/肌酐水平,为该类患者长期管理的有效模式。Objective To explore the effect of family doctor contract service mode on blood pressure control,urinary protein level,quality of life and medication adherence in hypertensive patients with microalbuminuria(MAU).Methods A total of 120 hypertensive patients with MAU were randomly divided into a management group and a control group,with 60 cases in each group.The management group received family doctor contract services,while the control group received routine community management.The changes in blood pressure,urinary protein level,quality of life and medication adherence were compared between the two groups before and after the intervention.Results After the intervention,the blood pressure levels of both groups decreased compared with those before the intervention(P<0.05).Compared with the control group,the blood pressure control effect of the management group was better,and both systolic and diastolic blood pressure were significantly lower than those of the control group(P<0.001).The urinary protein/creatinine ratio of the management group was lower than that of the control group,and the quality of life score was higher than that of the control group,but the differences were not statistically significant(all P<0.001).Conclusion The family doctor contract service mode can effectively improve blood pressure control in hypertensive patients with MAU,providing an effective mode for the long-term management of these patients.
