摘　　要：慢性缺血性疾病是一种常见的下肢病症,主要包括血栓闭塞性脉管炎、下肢动脉炎、下肢动脉血栓及下肢动脉硬化闭塞症等。根据世界卫生组织(WHO)的数据,全球约有2亿人患有下肢缺血性疾病。在中国,下肢缺血性疾病也较为常见。根据中国卫生健康委员会的数据,中国有超过1亿人患有下肢动脉疾病,其中大部分是下肢缺血性疾病。患者普遍存在不同程度的疼痛感,且经久不愈,严重影响患者的生活质量。因此,除痛疗法成为患者日常的重要环节。目前,关于下肢慢性缺血性疾病疼痛的临床治疗方案尚无完全统一的标准,已知的除痛治疗方法包括镇痛药物治疗、血管介入术、神经调节治疗、脊髓电刺激治疗、中医治疗及辅助治疗等。本文结合相关文献介绍并分析目前常用的镇痛治疗方法及其作用机制,并对其研究和应用进展进行综述,以期为临床下肢慢性缺血性疾病的除痛治疗提供参考。Chronic ischemic disease is a common lower extremity condition,which mainly includes thromboembolic vasculitis,lower extremity arteritis,lower extremity arterial thrombosis and lower extremity atherosclerotic occlusive disease.According to the World Health Organization(WHO),about 200 million people worldwide suffer from lower extremity ischemic disease.In China,lower extremity ischemic diseases are also very common.According to the China Health Commission,more than 100 million people in China suffer from lower extremity arterial diseases,most of which are lower extremity ischemic diseases.Most of them have lower extremity ischemic disease.Patients generally suffer from different degrees of pain,which is not cured for a long time and seriously affects the quality of life of patients.Therefore,pain relief therapy has become an important part of the patient's daily routine.Currently,there is no completely standardized clinical treatment plan,and the known methods of analgesic therapy include analgesic medication,vascular intervention,neuromodulation therapy,spinal cord electrical stimulation,Chinese medicine and adjuvant therapy,and so on.In this article,we synthesize the reported literature and present the results.This article combines relevant literature to introduce and analyze the current commonly used analgesic treatments and their mechanisms of action,and review the progress of these studies and applications.It is intended to provide some references for analgesic treatment of lower extremity chronic ischemic diseases in clinical practice.
