摘 要:目的综合分析消化内镜故障维修数据,为降低消化内镜故障率和制定维保措施提供建议及参考。方法收集华中科技大学同济医学院附属同济医院消化内镜中心2020年10月至2023年11月90余条超声内镜、电子结肠镜、电子十二指肠镜、电子胃镜和电子小肠镜3年内有效维修数据共计252例维修数据。从故障现象、形成原因、故障率、维修部位、维修部件、维修等级和维修费用等因素着手,多维度综合分析内镜故障影响。结果消化内镜故障现象主要为部件磨损/损伤(82.94%)、保护管套褶皱(73.02%)和漏水问题(67.06%)等9类。消化内镜故障形成原因主要为操作不当(92.86%)、维护不当(66.27%)和损耗老化(59.92%)等5类。在消化内镜故障率方面,消化内镜整体故障率为0.17%,其中电子十二指肠镜在所有消化内镜中故障率最高(0.74%)。在维修部位方面,维修部位主要集中于插入部(83.33%),其次为操作部(75.79%)和先端部(45.63%)。在维修部件方面,维修频繁的部件为按钮/旋钮/开关(75.40%)、钳子座(61.51%)和导光软管(57.94%)。在维修等级方面,消化内镜故障C级维修占比最多(60.71%)。在维修费用方面,超声内镜单次维修费用最高(168830.83元),电子胃镜维修费用总体占比最高(56.51%)。在次均维修费用方面,消化内镜故障维修次均维修费用最高的为电子小肠镜(258.66元)。结论基于历史维修数据的消化内镜故障分析有利于全面掌握消化内镜故障具体成因和维护风险点,可为医院降低消化内镜维修成本提供建议及参考。Objective To provide suggestions and references for reducing endoscopic fault rate and formulating maintenance measures by comprehensive analysis of digestive endoscopy fault maintenance data.Methods From October 2020 to November 2023,a total of 252 valid maintenance data in the three years including more than 90 endoscopic ultrasonography,electronic colonoscopy,electronic duodenoscopy,electronic gastroscope and electronic enteroscopy were collected.The endoscopic fault influences were comprehensively analyzed from fault phenomena,causes,fault rates,maintenance parts,maintenance parts,maintenance grade and maintenance costs.Results The endoscopic fault phenomena were mainly 9 types,including component wear/damage(82.94%),protective sleeve fold(73.02%)and water leakage(67.06%).The endoscope fault causes mainly include five categories,such as improper operation(92.86%),improper maintenance(66.27%)and loss aging(59.92%).The overall fault rate of endoscope was 0.17%,of which electronic duodenoscope was the highest(0.74%).The maintenance parts were mainly focused on insertion part(83.33%),followed by operation part(75.79%)and tip part(45.63%).The components with frequent maintenance were button/knob/switch(75.40%),pliers holders(61.51%)and light hose(57.94%).Of the maintenance grade,grade C maintenance of gastrointestinal endoscopy faults accounted for the largest proportion(60.71%).Of the maintenance costs,single maintenance cost of endoscopic ultrasonography was the highest(¥168830.83),and overall maintenance cost of electronic gastroscopy was the highest(56.51%),and mean highest maintenance cost of gastrointestinal endoscope fault repair was electronic endoscopy(¥258.66).Conclusion It is demonstrated that endoscopic fault analysis based on historical maintenance data is helpful to fully grasp specific causes of endoscopic fault and maintenance risk items,which could provide suggestions and references for hospitals to reduce endoscopic maintenance cost.
